Agreed! Social Determinants of Health include factors that can be the cause of health problems and they can lead to unnecessary hospitalizations, poorer health outcomes, and worse prognosis. Understanding and targeting SDOH is essential for developing long-term protections for communities disproportionately impacted by the healthcare system. We're proud to share that the countywide Community Health Assessment (CHA) has been completed! Typically conducted every five years, the CHA helps the Health Department document #WashtenawCounty’s health needs so we can work to address them together with partners and community through collaborative planning and activities. “Our Health Department’s approach to this process is really rooted in our commitment to health equity,” says Dayna Brimley, MPH, community health analyst. “Conducting a CHA is a huge lift that would not have been possible without the passion and commitment of our community members and organizations.” The CHA forms the foundation of the next step of this work: creating a Community Health Improvement Plan (CHIP). The three priority areas for the 2024 CHIP are mental health, access to health care, and access to healthy food. “The next part of our process is all about putting actionable goals in place to make real improvements in each of these priority areas - together,” says Brimley. Canadians deserve timely access to health services whenever and wherever they are needed. The Governments of Canada and New Brunswick announced a funding agreement of more than $313 million over 3 years to help accelerate efforts already underway in the territory to advance shared health priorities: Improving family health services, supporting and growing the health workforce, improving access to mental health and substance use services, including for Indigenous Peoples, and modernizing health care systems with health data and digital tools. As well, to help residents age with dignity closer to home with access to home care or care in safe long-term care facility, the Governments of Canada and New Brunswick signed another funding agreement of close to $117 million over 5 years.
