Our latest publication published in the Journal of Foot and Ankle Research Key findings: This study identified that community podiatrists are frequently managing foot ulceration in primary settings of varying severity and complexity. They are confident in their clinical skills but are not consistently applying assessment in-line with national and international guidelines. Common barriers to providing care of patients with foot ulceration were the financial capacity of patients to afford the clinical consult time and consumables required for wound care. There is an opportunity to provide structured education including clinical immersion to support community podiatrists to gain skills in assessment, triage and establish care pathways with their local HRFC. Podiatrists in regional and rural areas would particularly benefit from the use of telehealth to support their provision of evidence-based clinical care in this population.

Go home to get better. Something to remember for our loved ones and even ourselves. We seem to assume that facilities offer more. That hospital rehab is better, that skilled nursing facilities or "Rehab Facilities" offer a more advanced rehab journey. But what about motivation? AND what about the specificity of training. Facilities try to replicate cars, stairs, unlevel surfaces. However nothing will exactly replicate what a patient faces at home. Being able to train patients within their own environment can't be beat! Patient motivation when being surrounded by barriers they know they must overcome also is huge! No one is coming to bring you your meal, family cant wheel you down the ramp forever. Want to go to your favorite location? to church? to the park? That's motivation. All this makes me a huge advocate for home health. Cerebral Palsy and the Premature Brain Premature infants are at a higher risk of developing Periventricular Leukomalacia (PVL) which may lead to the development of Cerebral Palsy (CP). PVL is a softening of the inner part of the brain (white matter) from lack of oxygen or decreased blood flow before birth, during or shortly after. Injury to this highly fragile area causes cells to die, resulting in holes or empty spaces in the white matter. Damage to this area of the brain may be responsible for infants less than 32 weeks developing spasticity, cognitive impairment, vision issues, and ultimately the catastrophic diagnosis of CP. According to the March of Dimes, there are 2,158 operating birthing hospitals in the U.S. I am investigating how many of them offer occupational and physical therapy during postpartum admission to improve function after delivery. So far, I have 38 hospitals who offer postpartum rehab during the hospital stay: https://lnkd.in/gg27dP_Z In some of those hospitals, only 1 therapist is providing rehab to postpartum moms. If they get sick or leave, there is not a back-up therapist to replace them. That hospital no longer offers rehab during postpartum admission. If my initial goal was to help improve maternal outcomes with early access to rehabilitation services, then I am on a mission to find therapists who are providing their services directly in the hospital and home, regardless of whether the birthing hospital offers this as a universal option to families who choose to deliver there. This UCHealth article was a phenomenal read: there was so much intention to educate the audience on exactly what to expect during a rehab session in the hospital after c-section. 👩🏾🏫If you teach in an OT or PT program, this is an article to bookmark that covers a typical rehab evaluation after a major abdominal surgery that addresses social determinants of health in the acute care and cardiopulmonary assessment. 🩺If you are an acute care therapist, this is an article to bookmark to guide you on why certain interventions like an abdominal binder reduce pain in the majority of patients who are properly educated on how to wear one after their c-section. 📊If you are a researcher, this is an article to bookmark to investigate maternal morbidity rates between patients who receive rehab services in the hospital after c-section vs those who don’t. 🏥If you are the hospital administrator of UCHealth, there’s one minor error in this article that you want to correct before you use it to set an example to hospitals across the U.S. that are not yet offering rehab services to mothers after their major, open abdominal surgery: Correct this statement: “In the first program of its kind in Colorado, physical therapists at UCHealth Memorial Hospital Central and UCHealth Memorial Hospital North in Colorado Springs started doing visits in 2022 with each patient who had welcomed a new baby via a C-section.” Correction: UCHealth did not develop the first cesarean rehab program of its kind in Colorado. Since 2014, Heart of the Rockies Regional Medical Center in Salida, CO has offered rehab services in the hospital to every mother after both vaginal birth and cesarean section which was spearheaded by a women’s health physical therapist: Dr. Ann Croghan PT, DPT, CLC. Other than that, this article marks the first of its kind that prioritizes cesarean rehab and recovery for every family in the United States. ⭐️BPPV and PPPD in Older Adults⭐️ - Persistent Postural Perceptual Dizziness (PPPD) is a chronic functional dizziness that can be triggered by an acute vestibular event, however it may be triggered by anxiety or trauma. - Clients with PPPD have dizziness and/or unsteadiness, on most days, lasting more than three months, which is triggered by upright postures. - Benign Paroxysmal Positional Vertigo (BPPV) can be a trigger for PPPD and the research shows that in older adults, increased time to diagnosis and treatment of BPPV can increase the likelihood of progress to PPPD. This maybe be due to increased anxiety and fear of falling 😵💫 For Physiotherapists working with older adults this means that we need to screen for BPPV early and manage it appropriately. For clients with persistent symptoms following manoeuvres consider early Vestibular Rehabilitation to reduce the likelihood of progression to PPPD. ☺️ #vestibular #physiotherapy #bppv #pppd #dizziness

Aastha Hospice Dr. Abhishek Shukla - Today, the students from Ms Institute of Nursing participated in an enriching session on Geriatric Care led by Dr. Abhishek Shukla. The focus of the session was on recognizing the individuality of elderly patients, as each person faces unique health challenges. Dr. Shukla emphasized the need for a personalized approach, highlighting that understanding the specific needs of every elderly patient is key to effective care. The students were taught essential skills, starting with history taking to gather detailed information about the patient's past and present health conditions, followed by identifying symptoms and screening for metabolic illnesses. Special attention was given to the importance of consistently monitoring vital signs such as blood pressure, pulse, respiration, temperature, and SpO2 to assess the overall functioning of vital organs. In addition, Dr. Shukla explained the significance of performing a neurological assessment, specifically checking the patient's orientation to evaluate cognitive functions.
