Post-operative cognitive decline, or dysfunction (POCD), is a clinical phenomenon in which patients, particularly elderly individuals. experience cognitive impairment for some time following a surgery. It is newly defined as ‘delayed neurocognitive recovery’ if present within 30 days of surgery, or ‘postoperative neurocognitive disorder’ if present within 1 year after surgery. Although the mechanism behind POCD is somewhat unclear, evidence suggests that heightened inflammation in the brain following surgery is the more likely cause. Preoperative assessment of neurocognitive function and risk factor identification is crucial to ascertain the true extent of POCD and any causative link to anaesthesia and surgery. Ischemic stroke can complicate the recovery of cardiac surgery patients. 59% of cardiac surgery patients have a high-risk of stroke even though they do not have a history of Afib.
