While in senior management at the Greenville Health System years ago, we created a hospital-doctor affiliation system that is worth revisiting because it may help us understand our current medical care situation and a way to prioritize patient well-being going forward. If we define medical care as the perception patients have of our healthcare system, then our situation is dire as 70%-90% of the populace have an unfavorable opinion of their medical care. As we have established, lack of availability, corporatization, depersonalization, exploitation of non-clinicians and poor communications seemingly conspire to undermine patient well-being. And while all of these are important, none is probably more contributory than the clinical burnout experienced by doctors who have lost autonomy and the sense of meaning in their work. This brings us to the hospital-doctor affiliation classification I helped create, and one that grants various degrees of doctor autonomy.

When designing a clinical trial, understanding the standard of care is paramount. It’s not just about what treatments are available but how patients access them, their support, and their entire journey. Here are some quick thoughts on why this matters: 1️⃣ Patient Experience: Patients are not just data points but real people with unique stories. 2️⃣ Holistic Care: A clinical trial should provide care that matches or exceeds what patients receive in standard treatment. Otherwise, why would they sign up? 3️⃣ Decision Dilemmas: Choosing between standard treatment and a trial can feel like picking between going to your local restaurant you’ve been to 100 times or trying that new Thai place down the street - there is a possibility that the Thai place is better, but are you going to go against what you’re used to. Medical errors, a haunting specter in healthcare, are often met with unsatisfactory apologies. Our latest blog post, The Unspoken Truth: Why Apologies Fail to Rectify Medical Blunders examines the implications of this failure. Medical errors are not isolated events, but a web of interconnected oversights and systemic inadequacies. The aftermath goes beyond the immediate patient, degrading trust in healthcare systems and inducing 'second victim syndrome' in providers. The art of apology is more than an expression of regret. It requires acknowledging the mistake, empathizing with the patient, and assuring prevention of recurrence. Yet, fear of legal repercussions often results in insincere apologies, damaging the patient-provider relationship. We need a paradigm shift towards a transparent healthcare system that encourages open communication and genuine apologies. Empathy and accountability can bridge the chasm created by medical errors. Patients seek transparency and a genuine understanding of what went wrong, yet the current medical landscape often suppresses these qualities. The key is fostering an environment that encourages learning from errors and prioritizing patient well-being. In conclusion, the focus must shift from merely avoiding errors to learning from them, from hollow apologies to genuine accountability. An apology should heal the systemic flaws that caused the pain to begin with. Every patient deserves an apology that heals, not one that merely placates.
