Kudos to the U.S. Chemical Safety and Hazard Investigation Board for the thorough investigation of the Pryor Trust blowout, Oklahoma 2018, which resulted in five fatalities. The investigation resulted in several actions including the investigation of Automated Well Control. API - American Petroleum Institute published a bulletin in 2022, API 16H Automated Safety Instrumented Systems for Onshore Blowout Preventer Actuation. Safe Influx conducted a "Gap Analysis" between their system designed and built in 2019 and the 2022 API Bulletin, and determined COMPLIANCE. Mr. Klejst's presentation will focus on the key findings from investigations recently completed by the CSB. The Philadelphia Energy Solutions (PES) and Husky Superior investigations provide learning opportunities from incidents involving normal and transient petroleum refinery operations. The Intercontinental Terminals Company (ITC) highlights the importance of recognizing the hazards resulting from the formation of dead legs in chemical production operations. The presentation will conclude with a description of the loss of containment event at the Intercontinental Terminals Company, LLC (ITC) bulk liquid storage terminal.
