· ESD is effective for treating Early Gastric Cancer (EGC) and western experience for absolute indication (AI) ECG continues to grow. · Data in comorbid patients with more advanced EGC is limited (termed relative indication (RI). · We analyzed our experience in 157 EGC in 149 patients (mean size 30mm). · Over F/U of 52 months in a comorbid group (Charlson CI 4.2), between AI and RI EGC, 5-year Overall Survival (88.9 VS 77.9%) and DFS (83.2 vs 75.1%) were not significantly different. · No serious AE occurred after ESD. · RI EGC had more advanced histology with higher rates of poorly. differentiated lesions (37.5% vs1.8%, P < 0.001), lymphovascular invasion (LVI) (39.6% vs 3.7%, P < 0.001), SMIC (72.9% vs 15.6%, P <0.001), and ulceration (54.2% vs 13.8%, P < 0.001). · Despite these adverse features only 1 patient died of gastric cancer. · Hence ESD for RI EGC in the comorbid is safe and highly effective treatment, achieving DFS in the majority in long term FU.
