Many of us will know someone who got hurt in a healthcare setting - perhaps a patient who was ignored until their problem got critical or they died, or misdiagnosed for years, got an infection in hospital, fell, was given inappropriate medication, transfusion, injection or surgical intervention. Today is World Patient Safety Day and according to World Health Organization 3 million people die each year due to lapses in patient security. The examples are many. My uncle fell and hit his head. He was taken to the hospital in Birmingham but the hospital did nothing to check for internal injury and sent him home. After some hours, he became confused and started slurring his speech. On going back to the hospital, it turned out he had a concussion and he was bleeding in his brain. He had to be put in a comma for some time whilst he was operated. The whole thing left his disabled and unable to speak. A timely intervention would have prevented this. In another, my friend had endometriosis and suffered debilitating pain. When she had an episode, the pain paralysed her, literally. Her doctor kept diagnosing higher and higher doses of painkillers paracetamol and ibuprofen until she was taking the maximum possible doses of both - 1g paracetamol x3 per day alongside 800mg ibuprofen x3/day. After an extended period of this harsh medication regime, she developed stomach ulcers that meant even more frequent pain. The doctor could have prescribed medications that protects that stomach to prevent the development of ulcers but did not and this caused harm to this patient. Stories like this where physicians harmed their patients and endangered their life are far too common and in more than 3 million cases a year, people lose their lives every year as a result of such errors. The solution is simple. Speaking out and challenging a decision if you do not agree with it as a patient or as a family member or friend of a patient. We can prevent patient harm by asking questions. Let us do more of that to keep patients safer. Clearly communicate the patient's condition, ongoing care requirements, and any concerns. #TimeStamps: Include timestamps for each significant event, such as when the call was received, when the team arrived at the scene, and when the patient was transferred to the hospital. Accurate and detailed documentation is crucial in EMS, ensuring continuity of care, legal compliance, and effective communication between healthcare providers.
