Mass General Brigham has withdrawn its physicians from Steward Health Care's Holy Family Hospital campuses in Haverhill and Methuen, Massachusetts. The decision, communicated via email to staff, cites concerns about patient safety standards at the hospital. Steward Health Care responded, expressing regret and emphasizing the hospital's positive safety rankings. Mass General Brigham physicians will no longer perform elective and emergency procedures at Holy Family, and impacted patients are being rescheduled at nearby Mass General Brigham or community hospitals. The move complicates Steward's financial challenges, as it looks to sell Holy Family Hospital and three other facilities amid reported financial difficulties. Here's another in the "no surprise here" series on hospitals: "CMS updated its Overall Hospital Quality Star Ratings on July 31, assigning one star to 276 hospitals. Every year, CMS assigns star ratings to U.S. hospitals based on 46 hospital quality measures divided into five categories: mortality, safety, patient experience, readmission rates, and timely and effective care. Data reporting periods range from July 2019 to March 2023, depending on the measure. This year, 29 more hospitals received a one-star than last year." Looking at the reporting period for data collection, we know or perceive that a lot has changed in hospitals since 2019. I would guess more hospitals are functioning at a one-star rate today. Today, I cross two states' lines to receive my care in Boston at a 5-star hospital where I have received primary care and more for 31 years. In 31 years, I have only had two primary care physicians; stability and access I do not take for granted. While the bar is set very high for me, it saddens me how low it is for others, with many without access to care. Settling in medicine is not something I am willing to do, but I know others have no choice. Here are some of the highlights: "As part of this journey to open the virtual hospital, we determined a partnership would better facilitate and accelerate the shift in care from the hospital to the home, while delivering on the organizational objectives we set around patient quality and safety, provider satisfaction, and organizational sustainability. The ecosystem required for hospital-at-home is sizable and health systems will have to balance the desire to internally develop a comprehensive virtual hospital program against the size of the investment and know-how required to develop this model of care for patients at scale. Developing this ecosystem requires a myriad of services with reliable redundancies to deliver safe and efficient care across broad geographic and demographic areas. These services, routinely accessible within the hospital, are not available on-demand within each community. Developing this clinical ecosystem outside of the hospital therefore requires a rethink around developing highly reliable decentralized core clinical services including advanced nutritional services, wound care teams, occupational and physical therapy, mobile diagnostic and imaging teams, speech therapy, social services, and case management to name a few." Benefits include: Patient Preference: Many patients prefer receiving care at home due to the comfort and familiarity of their environment, which can positively impact their recovery experience. Safety and Confidence: The majority of surveyed individuals feel safe managing medications and care at home, indicating trust in home-based healthcare services. Faster Recovery: A significant portion of respondents believe that recovery is quicker at home, potentially reducing the overall burden on healthcare facilities. Cost Efficiency: Hospital-at-home care can lower costs by reducing the need for hospital stays, which often include higher overhead and resource expenses. I have personally experienced long, excruciating wait times at local community hospitals when my mother was still with us, and I can attest that this significantly impacts patient satisfaction. While data shows that Veterans tend to be more satisfied with the care they receive at the VA, it’s possible that they may not be fully aware of the range of services available at their local VA Medical Center for them to utilize! ****How ER Wait Times Compare: VA Medical Centers vs. Community Hospitals**** 1. Introduction: Emergency room wait times are a significant metric when it comes to patient satisfaction and overall healthcare delivery efficiency. Below is a comparative analysis of wait times at community hospitals and VA Medical Centers, illustrating where each stands in terms of providing timely care. 2. Community Hospitals (CMS Data): •Based on CMS data, the longest average ER visit times at certain community hospitals can reach up to 9 hours from arrival to departure. Example of the top 3 longest times: •Hospital A: 556 minutes (~9.3 hours) •Hospital B: 503 minutes (~8.4 hours) •Hospital C: 496 minutes (~8.3 hours) Reference: Community hospital wait times obtained from Becker’s Hospital Review. 3. VA Medical Centers (Data Snapshot): •Data from a VA Medical Center shows that patients in this system experience a considerably shorter wait time compared to some community hospitals. •Average Admit Delay: 199 minutes (~3.3 hours) •Percentage of patients boarded for >4 hours: 35.3% •Patients admitted in under 4 hours: 64.7% Additional details from the data show trends by: •Day of the Week: Longest delays on Sundays and Mondays. •Gender and Age: Most patients are between 51 and 64 years old, with males being the predominant group. Note: The data highlights that while delays exist, VA centers tend to manage patient flow more efficiently in terms of minimizing extensive delays compared to the longest waiting community hospitals. 4. Conclusion: While ER wait times at both VA Medical Centers and community hospitals can vary depending on several factors, the VA appears to maintain shorter average times compared to the community hospitals listed in the CMS data. A virtual wards operational framework has been published by NHS England, designed to help support consistency and the achievement of goals set out in the urgent and emergency care recovery plan and 2024/25 priorities and operational planning guidance around maintaining a virtual ward capacity of over 80 percent. The framework details a vision of What Good Looks Like for virtual wards, as well as scaling virtual wards, assessing progress, building an evidence base, recommended indicators and outcomes, through to operational and implementation requirements for integrated care boards. There are currently long waiting times for some NHS elective procedures. When patients have to wait longer for the care they need, it can cause both physical harm and mental distress to them, their families, and carers. The Government has set an ambition to reduce elective waiting times to less than a year by 2025. We understand that many factors contribute to long waiting lists for elective care.
