Faster Recovery: A significant portion of respondents believe that recovery is quicker at home, potentially reducing the overall burden on healthcare facilities. Cost Efficiency: Hospital-at-home care can lower costs by reducing the need for hospital stays, which often include higher overhead and resource expenses. Policy Support: Temporary programs allowing hospital-at-home care during the pandemic have been extended, reflecting positive policy momentum and recognition of its benefits. Mass General Brigham has withdrawn its physicians from Steward Health Care's Holy Family Hospital campuses in Haverhill and Methuen, Massachusetts.   The decision, communicated via email to staff, cites concerns about patient safety standards at the hospital. A provision within the Affordable Care Act (ACA) effectively banned ❌ any new physician-owned hospitals from opening or existing ones from expanding. This article from Health Leaders media interviews UMass Memorial on their award winning Hospital at Home program. The program's medical director spoke about the guidelines and guardrails that are followed throughout a home hospitalization. The practices that are outlined are standard across HaH programs as the availability of hospitalization from home grows. The article shares that: Patients are interviewed for the program after being admitted to the hospital through the ED, in a process that includes questions about their homes and social determinants of health. If they are admitted to the program, they are transported by ambulance back home and met there by a team from UMass Memorial that sets up the technology and helps both patients and family members get acclimated to the new devices and routines. The process includes assessing the patient’s surroundings, a key component in the health system’s quest to address health equity. "This allows us to understand them better,' Michaelidis says. 'We are laser-focused on health equity,' and that means identifying and recognizing the many societal, cultural, environmental, and technical aspects of one’s life that affect health and healthcare access. Everything from family environment, diet and exercise, job security, finances, transportation, and technological literacy is factored into how care is delivered. 'We have a unique opportunity to detect things before they become worse,' he adds. ...Once the patient is settled at home, the program kicks into gear. Virtual visits with the care team are held every morning, and in-person visits are scheduled at least twice a day. There are also regular 'huddles' with the entire care team, including physical therapists and social workers. Today, more than 300 hospitals in 37 states are providing home-based inpatient-level care programs. In November, 2020, the Centers for Medicare & Medicaid Services issued a temporary waiver of several Medicare Conditions of Participation that made hospital care at home possible, adding needed capacity during the pandemic. Facility staffing levels have not returned to pre-pandemic levels and likely never will. And there were many patients before and after the pandemic that could be safely and effectively treated in the home setting. Further, HaH programs are tailored in recognition of the specific social determinants of health affecting the patient, reducing readmits and emergency department usage. Congress should recognize this and make the Hospital at Home waiver permanent. See the link below regarding the process followed by HaH as demonstrated by Umass Memorial and Medically Home.

Here's another in the "no surprise here" series on hospitals: "CMS updated its Overall Hospital Quality Star Ratings on July 31, assigning one star to 276 hospitals. Every year, CMS assigns star ratings to U.S. hospitals based on 46 hospital quality measures divided into five categories: mortality, safety, patient experience, readmission rates, and timely and effective care. Data reporting periods range from July 2019 to March 2023, depending on the measure. This year, 29 more hospitals received a one-star than last year." Looking at the reporting period for data collection, we know or perceive that a lot has changed in hospitals since 2019. I would guess more hospitals are functioning at a one-star rate today. Today, I cross two states' lines to receive my care in Boston at a 5-star hospital where I have received primary care and more for 31 years. In 31 years, I have only had two primary care physicians; stability and access I do not take for granted. While the bar is set very high for me, it saddens me how low it is for others, with many without access to care. Settling in medicine is not something I am willing to do, but I know others have no choice.
