I overhear ER to floor nursing hand off quite often. My surprise? It's often a close version of the admitting HPI and A&P. Sure, the nurses take their own histories and have an idea of the plan. But your note is where it all gets pulled together. This has a profound power to frame the patient. "Transient Neurological symptoms" with a bunch of words that kind of suggest ( but don't really say outright) your concern for a stroke is much different than "Presumptive Acute CVA." Everyone knows what "Acute CVA" means and what they're supposed to do and look out for. I've seen a bad H&P cause confusion (and quite frankly sub-par care) for an entire hospitalization. So, don't overlook the importance of a good thorough history, and a good thorough H&P to frame the patient for the rest of the care team. ------- If you liked this post, consider reposting to share with your audience.
