Interventional nephrologist Dr. Neghae Mawla returns to the show to discuss endovascular AV fistula creation with our host Dr. Christopher Beck. Patients who come to see Neghae receive a standard vein mapping via ultrasound to determine whether they should receive an endovascular or a surgical procedure. If patients’ veins fit certain specifications, such as superficial location (cephalic, median cubital), large enough size (2-2.5 mm) and presence of large perforating veins (2 mm), then they are better candidates for an endovascular approach. It's cute. What is the "value" of this "value-based payment?" It sounds like the patients pay exactly what they would pay otherwise and possibly overpaying. The market rate for a non-contrast CT scan is $300-500. 24-hour urine collection is around $100-150 out-of-pocket. Let's see what a nearby private practice can do for the same patients operating in a surgical center. If you can provide lower costs and better outcomes compared to completion, that's "value." Otherwise, it's a gimmick.
As the need for value-based care strategies in the subspecialty space grows, we've seen very little progress in alternative payment models within urology. Urologic disorders, like kidney stone disease, can be viewed as a chronic disease with acute episodes -- this makes bundling payments a challenge. But our team at Vanderbilt University Medical Center leaned on our expertise within the MyHealth bundles program to create the *first ever* episodic bundled payment for kidney stone care, with zero out-of-pocket costs for most patients. We are very excited to share our initial experience with MyUrology Health, published in American Urological Association's Urology Practice.
