In 2024, CMS expanded service addressing health-related social needs. This was seen through the addition of the social determinants of health risk screening code, community health integration (CHI) services, and principal illness navigation (PIN) services. There are many tools available to support healthcare teams in connecting patients to local organizations and resources. Unite Us and findhelp.org are two of the resources our team leverages the most to help resolve patient needs. Findhelp.org compiles many local resources for cities across the country! Unite Us allows users to send referrals, track the status of referrals, and close the loop for addressing many social determinants of health. What tools does your team utilize to address SDOH needs?

Community Health Needs Assessments (CHNAs) are part strategic plan/part grounding rod, and provide a systematic process for determining health needs in a particular community or population and using results to spur community change. CHNA has long been best practice within the field of public health, but the process has gained momentum now that the Patient Protection and Affordable Care Act of 2010 (ACA) requires tax-exempt hospitals to do a CHNA every three years alongside stakeholders. CHNAs prompt those working to improve community health to consider local conditions—both community needs and assets—leading to more targeted, effective community improvement planning. At IP3, we believe that CHNA should catalyze community change, not just check a box. With IP3 ASSESS, communities can quickly access and interpret the quantitative data needed to complete CHNAs. In the lead up to Budget Day, Community Health First is recapping its important budget submission priorities. Priority #3 is “Inreach of Community Health into Emergency Departments”. Victoria’s emergency departments are currently under significant strain and are often used by people with complex care needs. Research shows that this complex care cohort accounts for 1.4 – 4% of all presentations, which equates to a minimum of $11M in emergency department costs alone. Unfortunately, the needs of these people cannot be met by emergency department staff, leaving many disconnected from proper support. Community health is uniquely placed to address this issue given the range of complementary services offered and its established community connections, and so we must have community health Complex Care Workers in emergency departments. For this to happen however, we need adequate funding from the Victorian Government and so we are calling on decision makers for support.
