To increase access to healthcare and improve outcomes for patients experiencing homelessness, a team from Valley Health Partners Community Health Center conducts street rounds. Clinicians carrying backpacks of supplies go into the community and offer care wherever it’s needed—from shelters and soup kitchens to encampments in the woods. The team uses Epic on laptops and mobile devices to document the care it provides, wherever that care takes place. Today, they’re increasing access to care for 80-140 people each month. The Healthy Opportunities Pilot is an innovative new approach to supporting social needs that impact health by using Medicaid dollars to pay for services related to transportation, nutrition, housing and interpersonal safety. Impact Health leads WNC’s participation in HOP. It's an initiative near and dear to our hearts here at MAHEC and part of a comprehensive strategy to integrate holistic and socially responsible practices into our healthcare delivery. Community health workers see the impact of HOP firsthand. Here's what Richard Montgomery, a community health worker for YMCA of Western North Carolina, has to say: "I was able to connect people who were coming home from prison to the HOP program. I delivered food boxes to these returning citizens who weren't eligible for food stamps. That was a big hurdle for a lot of people. I wanted to share this article about the plight that many of us will face who have aging parents and who live in the San Francisco Bay Area. Those who don't have means to pay for quality in home care or to pay for quality assisted living communities are left feeling that they will be a burden on their families. I think we need to build family and community support systems that are accessible and that have very few barriers to entry. Thoughts?

So much more to do to overcome the health outcome impacts of social determinants of health: poverty, housing insecurity, environmental racism, and food insecurity (among others!) CHCs still are the network resource to address these crises!
Nearly 60 years ago, visionary community activists and reform-minded physicians in the Civil Rights Movement established the first Community Health Centers to fight poverty, advance health equity, and change lives. In the process, they launched a national movement. Today, health centers are the largest primary care network in the United States, serving 31.5 million patients nationwide.
