A thought I’ve been having and sharing with my team and delagates at conferences. As emergency physicians we take pride in saving lives. But we save lives in that moment of time. What about the long term? Patients may come to us on zero meds and go back home on a dozen medicines. Those medicines are not going to “cure” them. Most patients brought into our departments have complications of metabolic syndromes which are curable and reversible - but not with medications. These medications would always keep your patients running in an infinite loop/vicious cycle of disease- medicine-disease. That’s just helping them manage to get through some more years of their lives. Is that why we become doctors? To Manage disease? Or to reverse them? Imagine if people reverse their diabetes, pre/diabetes, hypertension, hypothyroidism, fatty liver, PCOS etc. Just Imagine. And the cure is in front of us. Species specific lifestyle. That’s it. So many more people would live healthier longer lives. Cost of healthcare for the families will come down. Just think…. As physicians, we often encounter patients experiencing postprandial hypoglycemia, especially those with prediabetes or a strong family history of diabetes. This condition occurs due to a delayed early insulin response, leading to an initial blood sugar spike after meals, followed by an exaggerated insulin release that causes hypoglycemia a few hours later. What stands out in managing these cases is the role of diet and lifestyle. A shift towards smaller, fiber-rich meals, avoiding refined sugars, and focusing on weight management in obese patients can make a significant impact on blood sugar control. These patients need regular follow-ups, as they are often in the early stages of diabetes. Early intervention through lifestyle changes and ongoing monitoring is key to preventing the progression to full-blown diabetes. Additionally, metformin may also be used off-label for other conditions such as polycystic ovary syndrome (PCOS) and sometimes for metabolic syndrome. How it works: Metformin belongs to a class of drugs called biguanides, that works by reducing liver glucose production, enhancing muscle insulin sensitivity, promoting glucose uptake, and potentially inhibiting intestinal glucose absorption.
