With students back to school and respiratory illness season fast approaching, PRHC’s Emergency Department is expecting to see higher patient volumes and acuity, and longer-than-normal wait times. Patients in the ED are seen by a physician in order of priority based on the Canadian Triage and Acuity Scale. The scale ranges from conditions that are life-threatening to non-urgent conditions for which care can be safely delayed. While longer wait times can be frustrating, it’s important to understand that our healthcare workers need to prioritize patients who require more urgent care based on this scale.

#MedicareAdvantage plans were supposed to create an at-risk model that improved the value/quality/cost equation. It is striking to me that the last paragraph in the piece states that insurers "defend prior authorizations that are required for virtually all inpatient hospital stays as necessary to control costs and make sure care is effective and efficient." That tells me that they have not fully embraced a new mindset. They are fitting the square peg of value into a fee-for-service round hole. I suspect that this is because they have decades of bureaucratic infrastructure and mentality that would need to change and change is difficult. It is even more difficult if you're making money hand over fist if you don't change.
We need to change our system to decrease hospital admissions! This requires access to care, at all hours; not a message that says “ if this is an emergency, go to the hospital “. How does a patient know if it’s an emergency? That’s why they called the doctor. In addition, we need to focus more on prevention of illness, and not just treatment of illness. But that’s because there is no money to be made in prevention.

DID YOU KNOW | According to an article published in November 2023 from the Children's Hospital Association (CHA) , medication errors are the most common events reported to the Child Health Patient Safety Organization by the participating CHA hospitals. It was reported medication error rates are higher in pediatric patients at 31% compared to the adult patients at 13%. This higher rate was attributed to the complexity of the process in the pediatric patients because age and size need to be considered when calculating doses.
