Years ago, I was fortunate enough to have the opportunity to work with a company my organization acquired in Sweeden. One of my most memorable experiences, second to working with a team of truly amazing people, was seeing the Vasa. At the time I could not have imagined that this 400yr old warship could be viewed as a case study in organizational failure that rings true even today. Also, a sad reminder of how after 400yrs organizations today are still making the same mistakes on projects with deadly results. Here are some of contributing factors that caused the Vasa to sink some 400 years ago, and many are still alive and well in modern organizations: 1- Excessive schedule pressure 2- Changing needs 3- Lack of a documented project plan 4- Secondary innovations 5- Requirements creep 6- Ignoring the obvious

I like your list! It sounds like I should study that too. I've used some different language to indicate what poor leadership looks like, but much the same message. How do we resolve it? For me, it is to address those things within my team. Starts with establishing trust. Engaging the team with the plan, and risks. Then a consistency of execution. Thanks for sharing, Matt Ellickson!

Great insight, Matt. Like the analogy of what caused the ship to sink to what may cause organizations to fal. Keep it up.

It is an unfortunate truth I see too often.
