Medication Adherence: Strategies and Best Practices Improving the quality, safety and experience of care that patients receive is central to the Network’s mission and performance in Medicare Advantage (MA) value-based programs. Medication adherence quality measures are more heavily weighted in the MA Star Ratings and require medication adherence rates of 80% or higher for patients with diabetes, hypertension and cholesterol for each calendar year. To improve performance in these quality measures, the Network’s pharmacy team recommends four specific best practices for providers: 1. Know which medications are included in the medication adherence quality measures. 2. Partner with patients on purpose, side effects and cost. 3. For medications included in the medication adherence quality measures, extend the days’ supply and provide enough refills until the next intervention. 4. 𝗔𝗯𝗼𝘂𝘁 𝗛𝗯𝗔𝟭𝗰 Haemoglobin is a protein component of the red blood cell and carries oxygen around the body. When haemoglobin joins with glucose, it becomes glycated to form glycated haemoglobin A (HbA1c or A1c).
