As an NHS outpatient receptionist, there are many parts to your role that affect the patient's pathway and achievement of the 18 week Referral to Treatment standards. Things that you can do that can make a big difference are: 1. Making sure that the patient's demographic details are accurate and up to date 2. Making sure that there is a mobile telephone number available 3. Marking any language or interpreter needs 4. Outcoming the outpatient appointment putting the patient on the correct waiting list 5. booking an appointment 6. Checking that the 18 week RTT status has been completed by the clinician 7. Discharging a referral when appropriate 8. Understanding the different 18 week RTT statuses and when a clincian would use them When managing your clinics do you do the following? The lack of medical specialists in rural America is a big problem. One of the … primary causes of this shortage is something I like to call “White Knight Syndrome” (WKS). It’s a rampant disease and is hurting patients. Let me explain. WKS is caused when well intended healthcare systems falsely hope that a specialist is going to ride into their community, set up a practice for 30 years, and never leave. Sounds wonderful, but it only happens in story books. But wait Dr. P, why can’t I recruit a full-time specialist? Here’s why: 1. No one is going to agree to “only 10 days of ER call a month”. Change your outdated bylaws please and improve your job posting. 2. Most of us value quality of life over quantity of our paycheck. Your community is lovely, but we’d rather live in Nashville, Atlanta, Charlotte etc. No amount of money will change this fact. 3. Healthcare is a team sport. It’s really challenging to recruit additional staff to join you in rural communities. We can’t do this alone. What’s a solution you may ask? Innovative care models. Partner with companies like VirtuCare who implement outpatient clinics by leveraging telemedicine and periodic on site care. #employers tend to not value personal experiences, but they should: - I have lifelong experience navigating my own complex #medical needs. With 2 open-heart surgeries under my belt and regular consultation with several different medical specialists, not to mention personal experience with #mentalhealth #therapy and #physicaltherapy, I am a master at navigating the choppy waters of our labyrinthine medical system. I'm fighting in the same trenches as everyone else who has complex medical needs. Yet, I've been rejected for #patientadvocate and similar roles time and time again, being told that I don't have #clinical experience. If this sounds like bullshit, it's because it is. - I have been a #caregiver for multiple family members, helping with every aspect of their care, including medication compliance, #selfadvocacy with physicians and nurses, transportation, in-home care, rehab, etc. I haven't learned to merely master my own healthcare; I've helped others do the same. Yet, I've been rejected for multiple roles involving advocating for and serving caregivers, patients, and their families. It's because I don't have the proper job titles. If I were allergic to bullshit, I'd be sneezing all the time. These are two of many. Employers who think these experiences don't matter are elitist and full of mud. Medical Doctors have always held a special place in my heart. There was a time growing up where I wanted to be a Dr. I even had a one day stint at a public hospital in PMB in grade 11 and that’s when I knew for sure this was not my portion. In 2009, I became more aware of the plight of Drs and this broke my heart. I thought over the years that things had gotten better but it’s now 2024 and we are faced with Drs who have no jobs. How did we get here? I’ve been following the plight of the Drs in KZN since they marched to the Department of Health headquarters in PMB in February. The department promised them jobs and I thought that was sorted. In early March, I met some Drs who are in the group of the unemployed. I shared what I had been reading and hearing and they said all that was false. They were still without work. It is now April and the Drs are still not working even though they were promised to be placed by 1 April. Since 2 April, the Drs have been staging a sit-in at the Health headquarters waiting for answers and appointment letters as per government’s promises. I was unable to join and support the sit-in last week but my heart was with them and I kept in contact with how things were going. Yesterday, I was finally able to join them. We hear about a shortage of Drs in public hospitals but there are 100s of Drs not placed which means there are 100s if not 1000s of citizens across our province not receiving medical care. This is a disservice to our people especially to the essence of SA’s “Batho Pele” principles. The current government is not putting people first from the patients who need medical treatment and to the Drs who need to care for themselves and their families. This is not just a public health crisis but a leadership and management crisis. As RISE Mzansi we believe that we need new leaders in the public health system. We need new leaders who are empathetic, ethical and accountable. We cannot continue like this. We need to RISE and say enough is enough! We need to RISE for new leaders!
