This 20-year follow up report reveals that while rates of uninsured have declined, we have not yet reduced health disparities in the United States. Until we recognize the multi-layered impact of community assets, politics/legislation/health policy, systems of care, healthcare provider and individual factors and accordingly implement multi-level approaches with measurable accountability across each layer as well as make visible the invisible, change will continue to move slowly.

Ending Unequal Treatment: Strategies to Achieve Equitable Health Care and Optimal Health for All, initiated and published by The National Academies of Sciences, Engineering, and Medicine does an amazing job consolidating some of the best thinking on how to improve overall US population health by emphasizing the need for health equity across our diverse communities. How's this for a call to action, "The way the U.S. health care system is organized, financed, delivered and held accountable does not live up to its potential. Despite being the country that spends the most money on health care among all high-income countries, the United States has some of the worst population health outcomes and is far from achieving optimal health for all. Its health care system is broken and by its very design, delivers different outcomes for different populations resulting in persistent and profound health care inequalities. The inadequacy of the system disproportionately affects minoritized populations, with stark racial and ethnic inequalities in life expectancy at birth, maternal and infant mortality, and many chronic diseases."

Lives Cut Short from the Institute of Health Equity, has confirmed that a million people in 90% of areas in England lived shorter lives than they should between 2011 and the start of the pandemic. The new findings from the IHE add weight to its two reviews of health inequalities in 2020 (Marmot Review 10 years On Review and COVID-19 Marmot Review): that the cumulative impact of regressive funding cuts (which hit poorer areas more), associated with austerity, contributed to life expectancy failing to increase, and actually falling for women in the 10% of poorest areas, and health inequalities widening. Well established evidence confirms most of our health is determined by our social circumstances and NHS care only accounts for 20% of population health. https://lnkd.in/enruQbnk
