I work very closely with our physicians at Topography Health (and have worked with physicians my entire career at both Flatiron Health and Epic). It's one of my favorite parts of my job. It’s also the best way to stay close to the challenges they face in today’s complex healthcare market. And while it is true that physician burnout is on the rise (for very valid reasons), the majority of doctors that I know don’t want to leave patient care. So, you can imagine how I feel when I see services emerging that help physicians “pivot” their careers away from practicing standard of care medicine and into only industry-sponsored research, as though it is a separate field entirely. Don’t get me wrong—industry-sponsored research is incredibly complex and requires different tools and support, but it does not have to be separate from practicing medicine, and in fact I don't think it should be. Enrolling in a trial is an intimate care decision between patient and provider, and offering trials in your organization as an extension of standard of care can be the next step in your patients’ journey. Additionally, from a business perspective, offering clinical trials inside your organization—whether you go it alone or seek the support of external partners—keeps the patient close to their trusted medical team and prevents unnecessary transitions of care. As a medical specialist, I am reminded every day of the profound impact a fresh set of eyes and a compassionate heart can have on patient care. Each individual who walks through our doors brings with them a unique story, a set of symptoms, and a personal journey towards healing. It is our responsibility, as healthcare providers, to approach every patient scenario with a renewed sense of curiosity, empathy, and openness. When we look at each patient with a fresh set of eyes, we create space for discovery and innovation. No two cases are exactly alike, and by remaining open to new possibilities and alternative perspectives, we can uncover solutions that may have previously gone unnoticed. Every patient encounter is an opportunity to learn, to grow, and to push the boundaries of our understanding. In the fast-paced world of medicine, it can be easy to fall into routine or to rely on past experiences to guide our decisions. The decline of physician autonomy When discussing the reduction of physician autonomy, it's essential to understand the fundamental dynamics of employment as outlined by the United States Department of Labor. Employment inherently implies a level of control where employers dictate the manner, timing, location, and specific duties of their employees. This principle of control extends into the healthcare sector, significantly impacting the practice of medicine. Medicine, however, cannot be distilled into mere protocols or guidelines—it embodies an art, requiring a personalized approach to applying medical principles to each patient's unique circumstances. Yet, physicians often encounter constraints imposed by their employers, infringing upon their professional judgment and the nuanced application of medical knowledge. These restrictions challenge the essence of medical practice, compelling physicians to adhere to standards or expectations that may not align with the best interests of individual patients.
