The Piper Alpha disaster reminds us how important the priority of safety management decisions is, specifically in crucial moments. How would the accident have gone if the other HSE management in the oil platforms had shut the process off before the accident propagation? Another lesson we learned from the disaster is that unless all personnel who were rescued alive were those who not followed the emergency response procedures! this accident bitterly taught us that the situation could have changed during the accident and can't be predicted unless you conduct consequences modeling using mathematical models and utilize all other tools to mitigate the risk as well as being ready for any events by training via distinguished emergency planning.
