Lack of geographical access to healthcare is a huge problem in Texas. Over half--60%--- of rural hospitals have opted to just stop offering care to pregnant women. It would be one thing if there was at least access to care through midwifery services, so that women could receive care in their communities and be sent to tertiary care centers if they need it. But Texas doesn't allow that, because we're still in the dark ages. We NEED full practice authority for NPs and CNMs to help bring care to the people where they are--that's the whole reason CNMs and NPs started practicing in the first place. Integrating CNMs and NPs into the healthcare system as independent practitioners helps develop a network of care that identifies people at high risk of complications and gets them in the right door to see the right people. I know there will be push-back--what happens when a mother needs an emergency CS but she's laboring with a midwife who doesn't do them? I'll tell you--- Someone will be there to call for assistance because it will be recognized and acted on sooner. A skilled person will be there to say, "Hey, this is not right, we need a medical assist." And if they're not being forced to practice underground, they can get the ball rolling sooner. So maybe, now there's a CHANCE to save them, where there was no chance before, because if they're practicing underground, there's no way to safely refer and give report to a hospital--they'll be prosecuted. So instead, they send you in your own car to the nearest hospital and pray. Then there's a delay in care because the hospital doesn't have the expertise, equipment, or experience to recognize what's going on and they have to try and figure it out from scratch before transferring to the right place. Delays cost lives. Maternity care deserts kill. Why does one of the richest countries in the world allow this kind of third-world nonsense?
