You go to hospital, you want to be safe, you fly on a plane you want to be safe, but sadly the news recently is filled with accounts of Staff either being too scared to even raise patient safety issues, or staff at Boeing being ignored or victimised for raising safety concerns. The EU Directive on this topic makes a number of recommendations including the use of anonymous reporting channels to protect the indentity of the staff members. It's crucial to prioritise safety monitoring in healthcare systems, especially after the recent suspension of over 30 staff from an NHS hospital over serious allegations. Independent safety monitoring systems play a significant role in ensuring patient well-being and maintaining medical integrity. Establishing and consistently adhering to rigorous safety protocols is the first step in creating a safe healthcare environment. Regular training and updates are essential in maintaining high standards of care, preventing deviations that could compromise patient safety. Systematic and routine audits are another proactive measure that can help identify irregularities or misconduct. Regular internal and external audits can uncover issues before they escalate, providing an opportunity for timely intervention and correction. Safety monitoring is crucial to ensure transparency and accountability which is fundamental in creating a positve caring environment. Please share and sign our petition for an Independent Patient Safety Commissioner. Unsafe staffing levels, overwhelmed hospitals/GP clinics, and the inability of all of our health entities to identify, mitigate and properly monitor harm, means safety cannot be assured. This role essentially would be bringing something new to our health system- independent scrutiny which is desperately needed. The PSC would be a proactive force, someone at the helm ensuring that there is no siloing of information within existing agencies, and that there is a timely, cohesive and concerted response to evidence and patterns of harm. The PSC would hold a mirror up to the health system to focus on reducing the harm that is happening, and in the long term hopefully reduce the number of complaints to the HDC and treatment injury claims to ACC. # We need a Patient Safety Commissioner so that no New Zealander, no health consumer has to fight to prevent harm in the health care system. 🆕 We've published the first in a series of reports that look at NHS workforce and patient safety. Our investigation has found that not involving temporary NHS staff in serious incident investigations may undermine ability to improve patient safety. From analysis of 30 NHS serious incident reports, we found 18 cases where temporary staff were not involved in the subsequent patient safety investigations. The main risk if temporary staff are not included is the loss of vital information and learning, raising the possibility of a reoccurrence of patient harm. Our safety recommendations to NHS England aim to increase the engagement of temporary staff in investigations and provide them with more support. We also include prompts in the report for healthcare providers to help consider how they involve temporary staff in patient safety investigations.
