The American Heart Association predicts heart failure will affect more than 8 million Americans by 2030. During American Heart Month, we are highlighting ways Humana has identified opportunities to improve health outcomes and reduce costs for those living with cardiovascular disease. According to a recent Humana Healthcare Research study published in the Journal of Managed Care & Specialty Pharmacy, heart failure patients who delayed filling new prescriptions by more than 30 days after hospital discharge had a 65% increased risk of rehospitalization within 6 months. Delayed filling was also associated with 49.5% increase in average all-cause medical costs. These findings can support further development of post discharge clinical program to improve timely medication adherence after a heart failure event. 1.3 billion people worldwide are expected to be living with diabetes by 2050. To help decrease that projection, diabetes education must be available to all patients diagnosed with the condition. My Diabetes Tutor (MDT) partners with physicians and practices to help patients better control diabetes through virtual Diabetes Self-Management Education and Support (DSMES). Patients receive personalized education and support on the core pillars of diabetes management, as established by the American Diabetes Association (ADA). The results are impressive: MDT’s patients achieve a 1.4% average reduction in A1C and a 10-point decrease in LDL.
