Launch event! There is growing awareness that community #wellbeing is something for which the NHS does not have sole responsibility. Consequently, research and development funding is increasingly seeking evidence of #partnership and #collaboration between statutory services, the third sector, the private sector and academia. The Centre for Healthy Empowered Communities acts as a hub for these sectors, and via collaborative projects is ideally placed to provide an evidence base and solutions to support wellbeing, prevent ill-health and help address health inequalities. Join us on Teams on the 5th June for an interactive morning to understand: - the current state of play regarding health inequalities and outcomes - how doing things differently can help - how collaborative community based action research, via co-creation can provide solutions - how the Centre for Healthy Empowered Communities can help you develop your idea into a successfully funded #project. Alongside this you will hear from a range of expert speakers including Dr. Leonora Weil from the UK Health Security Agency, who is also Director of the NHS Legacy and Health Equity Partnership. During the interactive sessions, you will be: - sharing your #ideas and concerns around #HealthInequalities - talking about improving connections and breaking down silos - discussing the help you need in order to take positive action. CONGRATS to Contexture and Quality Health Network (QHN)! Beyond Lucid Technologies has been proud to be the first (and still ONLY) #MobileMedicine company to *automate* discrete health information sharing between #Fire #EMS #Ambulance #NEMT #CCT #CP_MIH agencies, and both of Colorado's #HIEs! Such automation and innovation extends back to 2019 for QHN and even earlier for the former CORHIO (a Contexture organization) - Follow @ContextureHIT. -- Stuart Mills, Tim Nowak, and Mike G. partnered to start the ball rolling at Centura Health (CASTLE ROCK ADVENTIST HOSPITAL), the first hospital IN THE UNITED STATES to receive real-time data from an ambulance (using MEDIVIEW, and we remain super proud of that milestone achievement!). -- Reuben Farnsworth, David Jepsen, and the Delta County Ambulance Dist team accelerated it further, showing how #CommunityParamedicine's economics are bolstered by data-driven collaboration over time. H/T Rocky Mountain Health Plans -- Now, it seems that agencies across Colorado will be able to envision a path to engage both systems at once; and then, via MedAllies working with Beyond Lucid Technologies, to interoperate with HIEs across the U.S.A., no matter what #ePCR they use! Alice Harvey MSN RN, Brandon Daruna, and the Eagle County Paramedic Services team have been pushing for borderless data interoperability for a long time. Imagine the next steps and implications for #MIH if longitudinal data and POLST / MOLST / Advance Directives could be accessible to field responders in real-time, at the patient's side, during a moment of crisis. Let's make #interoperablity an SOP for Mobile Medicine! Marc Lassaux, Melissa (Rutala) Kotrys, Scott Brown, Maria D. Moen, A. John Blair, III, MD, David Moffitt, Amber Viitanen, MPH, Donnie Woodyard Jr., MAML, NRP
Contexture and QHN have affiliated to unify health information exchange in Colorado. “When Contexture and QHN began the affiliation process, we realized just how vital this coming together would be for the state of Colorado and beyond. Our participants desire more than standard HIE services to meet their organizational objectives, and by affiliating with QHN, Contexture will continue to enhance our solution offerings to keep up with essential community health needs.” - Melissa Kotrys, CEO of Contexture “Affiliating with Contexture is an important step forward for QHN’s participants as we work to provide the best possible HIE services, putting data into the hands of care providers when they need it. Our team is excited to provide enhanced services tailored to the needs of the medical, behavioral and social service providers we serve.” - Marc Lassaux, Executive Director and CEO of QHN. Empowering Change in Public Health: My Journey and Lessons Learned/Learning As I reflect on my journey in public health, I am reminded of the power of passion, perseverance, and collaboration. From my early days as a student to my recent role at Afri-health, I have been driven by a desire to make a difference in the lives of others. Early Beginnings My interest in public health began with a simple question: "How can we create a world where everyone has access to quality care?" and “Why treat the disease when you can prevent it?” This question sparked a journey that has taken me from the classroom to the field, working with communities, organizations, and individuals who share my passion for positive change. Lessons Learned/Learning Along the way, I have learned valuable lessons that have shaped my approach to public health: - Community engagement is key: True change happens when we work with communities, not just for them. - Innovation requires risk-taking: We must be willing to try new approaches and learn from our failures. - Collaboration is essential: Together, we can achieve far more than we can alone. - Equity and access are non-negotiables: We must prioritize the needs of marginalized communities and work towards inclusive solutions. My Experience at Afri-health so far My time at Afri-health has been a highlight of my journey. Working with a talented team, we developed and implemented programs that is improving health outcomes, sustainable in long term, and is saving lives. I learned/learning the importance of: - Data-driven decision-making - Strategic partnerships - Adapting to changing circumstances The Future of Public Health As we look to the future, I believe we must prioritize: - Technology and innovation - Community-centered approaches - Equity and access - Global collaboration Lastly, My journey in public health has taught me that change is possible when we work together towards a common goal. Let us continue to inspire and empower each other to create a healthier, more equitable world for all. Feel free to share your own stories and lessons learned in the comments! I would love to hear from like-minded individuals. #publichealthcare #inspiration #collaboration #innovation #PublicHealth #GlobalHealth #HealthEquity #CommunityHealth #HealthcareAccess #PublicHealthMatters #HealthcareTransformations #WellnessAndPrevention #HealthcareLeadership

Nottingham East PCN from September 2024 are delivering Community Hub events weekly. From the events we hope to provide awareness of Health and Social Care services, as well as build individuals health literacy to encourage patients to feel confident in managing their own health conditions. We are hoping that our community hub events: Build Trust: Hosting events in the PCN 6 and familiar settings will help build trust between healthcare providers and the community, encouraging people to seek care who might otherwise be hesitant. Education and Awareness: These events provide opportunities to educate the community about important health issues, encouraging healthier lifestyle choices and increasing awareness of available services. Cultural Sensitivity: Community-based care can be tailored to meet the cultural and social needs of the population, improving the relevance and effectiveness of health interventions.

CCHA’s Community Impact Manager Kim Rawlinson recently provided commentary on cross-sector collaboration in Health Services Research. She writes, “Given the longstanding health disparities in our country, organizations…must be radically aligned, and collaborative efforts must be transformed to achieve health equity. We must dismantle and reimagine many of the institutional structures and policies that are inequitable and perpetuate health disparities… The time is now. We cannot afford to wait to make the necessary changes to move our country in the right direction, and we cannot wait for the next emergency to build aligned processes.” Read more: https://loom.ly/vp1eiyM

We have published the results of the National Engagement on Digital Health and Social Care, a project undertaken in partnership with the Department of Health - Ireland and the Health Service Executive. The project aimed to understand the opinions and attitudes of the Irish public and professionals on a digital approach to health and social care. We surveyed over 2,000 members of the public and over 1,000 professionals working in health and social care. We also held focus groups with members of the public, including representatives from minority groups and people who use disability services, and professionals working in health and social care. The results showed that the public are ready to engage digitally to help manage their health, while professionals think digital care is important to empower patients as partners on their healthcare journey but are concerned that some members of the public may not be ready to interact with them digitally. The findings from this engagement will inform our work in health information, as well as informing the development and provision of digital health systems and services in Ireland by Department of Health - Ireland and Health Service Executive. The Biden administration published an important trio of publications affirming the importance of multisector collaboration across social care, health, and public health systems, and we at DASH could not be more pleased. Together, these pieces offer unequivocal support for the notion that social determinants of health (SDOH) are pivotal in determining individual health outcomes, and that multisector collaborations are key for healthier communities and individuals. : : : #community #data #health #healthcare #data4health #wellbeing #publichealth #equity #healthequity #sdoh U.S. Department of Health and Human Services (HHS)
