This fatal incident reminds us how crucial of developing proper planning before lifting and shifting material by mechanical equipment such as forklift.Here there were some causes behind this incident. 1:- Safe working load of forklift not checked(swl). 2:- Load was not below as it should have while shifting. 3:- Incompetent operator. 4:- unavailability of flagman. 5:- vehicle path was uneven. 6 :- untrained victims. 7 :- slippery surface. 8 :- Lack of training and supervision. 9:- imposed load beyond swl.

Always adhere to the forklift's Safe Working Load (SWL) and ensure only trained and certified operators are in control. Regular training updates are essential to stay informed about new safety protocols and equipment. Before use, check the forklift for visible damage or leaks, confirm all controls, lights, and warning devices are operational, and ensure tires are properly inflated and in good condition. Maintain a clear work environment, watch for obstacles, and be mindful of floor conditions like wet or slippery surfaces. Stay alert for pedestrians and other vehicles. Never exceed the forklift's load capacity. Ensure loads are stable, balanced, and secure. Keep loads low and tilted back while moving. Always wear a seatbelt if available, drive safely, and avoid sudden stops or sharp turns. When parking, lower the forks, turn off the engine, remove the key, and engage the parking brake to prevent movement.
