As we wait for the #NHS New Hospital Programme Review, what if it provided a clear, definitive roadmap for the major hospital redevelopments the Government plans to prioritise? Imagine these projects programmed over, say, 20 years – couldn’t that list be embedded into policy? If so, wouldn’t this simplify business cases and capital processes, shifting the focus to *how* to deliver the plan, rather than debating *whether* to pursue each project individually? This feels like a real opportunity for the Government to cut through unnecessary debate and delay – driving faster progress on critical healthcare infrastructure. What’s your view? Matthew Custance The current system for deciding which/ when/ where to build new hospitals in the UK clearly isn’t working, and needs to change. PFI in my view didn’t work either, as it bundled building, FM and other services into complex contracts and the PFI consortia quickly became good at stealing sweeties from tge kiddies letting contracts. Has anyone done a comparison of the end to end process for planning and building a new public hospital in UK vs a range of other developed countries with either social health insurance or Bevan systems? My hypothesis is that in the UK there is a much greater degree of political and treasury involvement, leading to delays, decisions which are sub-optimal at a local level, and excess time and money spent planning projects.

Does investing in assets drive hospital revenue? Is this affected by socioeconomic settings (Rural v Urban)? Here are some more insights from the 2021 CMS Hospital Provider Cost Report... Restricting myself to California hospitals. Out of the 412 listed hospitals, only 332 had valid values for Total Assets and Net Patient Revenue. Outliers were removed using the Inter-Quartile Range method. Plotting a polynomial regression line, there is a point where higher total assets are accompanied by LOWER patient revenue. Despite there being fewer rural hospitals (69) compared to urban ones (263), rurally situated hospitals (in RED) do not necessarily invest in fewer assets. So spending more money on land and other infrastructure does not guarantee higher revenues. Like most businesses, success (and profit) is driven by the efficient use of existing resources. #healthcare #analytics
