There are a number of arms to this issue. Undoubtedly we require cultures and workload management structures that support collegiality, inter-professional collaboration, and institutional challenge and accountability that leads to the kind of change that optimizes excellence in patient care. The second, and not to be considered secondary in priority to the former, requires a concerted understanding and integration of the ‘experience’ of professional role transition for healthcare professionals. The experience of transition significantly influences and impacts on the new professionals assimilation into their new work context.

WORLD PATIENT SAFETY DAY ; SEPTEMBER 17TH 2023 Every September we take time to emphasize on patient safety. This year's theme is Engaging the patient and the family for safe care, while the slogan is ELEVATING THE PATIENT'S VOICE. SQHK is not left behind in this effort. We drum the beats for safe care and advocate for patients being heard, getting safe care and being engaged in the care they recieve. How can we elevate the patients voice in our clinics, wards, in emergency care, at community, in chronic care, in acute care? What are some of the practical ways of having the patients empowered to be enablers of desired outcomes? How do we win together, having care goals that are shared between the patients and clinicians? How do we have shared decision making in healthcare, listening to the prefferences of the patients and the families? How do we empower the families to support thier loved ones at the comfort of thier homes? How do we support end of life care offering emphathy, comfort, dignity and information required? #engagingpatientsforsafecare #qualityhealthcare #patientcentriccare Caleb Mike Mulongo, Rhoda K., Frank Wafula, Daniel Kiura, Lydia Okutoyi

While Patient Safety Week primarily focuses on hospital settings, we want to extend the conversation to the crucial period after discharge. At Comfort Keepers, we understand that patient safety doesn't end at the hospital doors. Our dedicated caregivers play a pivotal role in ensuring a smooth and secure transition to home care. We work closely with discharge planners, healthcare professionals, and families to provide the support needed for a safe and successful recovery at home. Our services encompass medication management, assistance with daily activities, and monitoring vital signs to prevent complications. We prioritize patient safety by offering a reliable presence in the home, reducing the risk of falls, medication errors, and other post-discharge challenges. This Patient Safety Awareness Week, let's remember the importance of keeping patients safe not only within the hospital walls but also as they recover at home. #PatientSafetyWeek #HomeCare #PatientSafety #Caregivers #Healthcare #RecoveryCare #ComfortKeepers #PatientCare

EMPATHY: The heartbeat of our care. — The best of care is not just about treating symptoms; it is about treating people. It 𝘂𝗻𝗱𝗲𝗿𝘀𝘁𝗮𝗻𝗱𝘀 𝘁𝗵𝗮𝘁 𝗲𝘃𝗲𝗿𝘆 𝗽𝗮𝘁𝗶𝗲𝗻𝘁 𝗶𝘀 𝗮 𝗽𝗲𝗿𝘀𝗼𝗻 𝗳𝗶𝗿𝘀𝘁 𝗮𝗻𝗱 𝗳𝗼𝗿𝗲𝗺𝗼𝘀𝘁. It recognizes that behind every illness, there is a person with dreams, fears and a story to tell. And in the hands of an empathetic healthcare provider, that story becomes a journey of healing and recovery and can turn a daunting hospital room into a sanctuary of hope. — In a world of data, diagnostics, and technology; it is easy to forget the most potent medicine of all: empathy. — 𝗔𝘁 𝗦𝘂𝗻𝘄𝗮𝘆 𝗛𝗲𝗮𝗹𝘁𝗵𝗰𝗮𝗿𝗲 𝗚𝗿𝗼𝘂𝗽, 𝗲𝗺𝗽𝗮𝘁𝗵𝘆 𝗶𝘀 𝘁𝗵𝗲 𝗰𝗼𝗿𝗻𝗲𝗿𝘀𝘁𝗼𝗻𝗲 𝗼𝗳 𝗼𝘂𝗿 𝗵𝗲𝗮𝗹𝘁𝗵𝗰𝗮𝗿𝗲 𝘀𝘆𝘀𝘁𝗲𝗺 that empowers our nurses into guardians, our doctors into healers and our patients into warriors.
