The impact of #HoFH extends beyond health, affecting self-perception, education, and employment opportunities. Despite the challenges, many individuals and families affected by HoFH demonstrate resilience and adaptability. Empowerment through education, self-care practices, and advocacy can contribute to better psychosocial outcomes. Recognising these challenges underscores the importance of collective action in raising awareness and supporting research efforts. Though untreated HoFH may limit life expectancy on paper, it doesn't have to limit the impact of our compassion and advocacy. Join us and participate in our logo contest, a meaningful opportunity to contribute to the cause and make a positive difference in the lives of those affected by HoFH. Use your creativity to help change it! Bring joy and excitment to our world. Join the #logo competition and help unite the global HoFH community. https://lnkd.in/eb7cy7Wi #Unite4HoFH #RareDisease, #UseHeart to #KnowHoFH and #FindHoFH #Maythe4thbewithyou #Impact #QoL

"With patient navigation, [it's] that extra layer of checking in with somebody, making sure that all of their socioeconomic needs are met, and their mental health needs are addressed within a family planning visit, and any other need that may arise that may contribute to their physical health that the provider is not able to address at that visit. " In our latest video, learn how the Konza Prairie Community Health Center, a #TitleX funded service site, partners with a local agency to provide patient navigation services that address maternal health needs and help to deliver #WholePersonCare in rural Kansas: https://lnkd.in/gfHa4wEU

It is only now, through concerted efforts of people with direct experience themselves and allies, that evidence and broader interest is starting to be paid to this critical issue. LELAN is commited to ensuring people with lived experience are centred or leading these conversations and related advocacy efforts in Australia. In seeking to activate people from the grassroots individual experience level through to collective systemic focused action we know that understanding the perspectives of clinicians, academics and interested others will help us to shape the messages and recommendations for how we best proceed strategically. Ahead of a House of Lords debate on the state of #SocialCare and the case for a comprehensive plan, we sent this briefing to Peers, describing the key findings from our research and analysis on social care over several years. We believe social care needs an exercise similar to the Darzi review for health. https://lnkd.in/eJdvpYRy

THE FIRST EVENT IN THE FRAMEWORK OF THE #project "Navigating Life with Dementia", financed by the European Union and the Government of Montenegro, took place today in the city of Nikšić, with the support of partners from NGO Impuls. The #kickoff event was attended by the representatives of the municipality Nikšić, employees of the home for senior citizens and the mental health center, as well as social workers, psychologists and volunteers of the project. At the event we discussed the current state of the project, challenges, advantages and outlined path of activities that will be implemented during the next two years. All partners present emphasized the importance of understanding and recognizing dementia, which would allow a new perspective on this disease (especially progressive in the 21st century). Partners and colleagues were also eager to listen to the volunteers, who perform honorable work and share "first-hand" experiences, which is the key to planning further work. Associated partners are: municipality of Plav, Red cross Nikšić, municipality of Berane, Center of social work for municipalities of Berane, Andrijevica and Petnjica. #dementia #psychosocialsupport #caregivers #communitysupport
