That's why we have dedicated Medicaid specialists like Denise at the ready to help support your patient populations with any barriers to care, such as: + Transportation + Food insecurity + Access to knowledge and community resources By removing these barriers, we help patients start to prioritize preventative healthcare.

Timely access to high-quality primary care is important for public health - but it also is all too often not equitable. That is why the Centers for Medicare & Medicaid Services #CMS announced a new model - making sure primary care providers in eligible Accountable Care Organizations (ACOs) can treat people with Medicare using innovative, team-based, person-centered proactive care - brining high quality care closer to or into communities and into the home of patients. https://lnkd.in/eKp7TvPt

Now out in JAMA Health Forum, we studied nationwide availability and uptake of dual-eligible special needs plans (D-SNPs) with exclusively aligned enrollment (receive Medicare and Medicaid benefits through the same plan or affiliated plans within the same organization). Among 2.7 million full-benefit dual-eligible beneficiaries nationally, we found that 38.9% resided in counties with exclusively aligned plan enrollment available and that 11.9% enrolled in such plans. -with Eric Roberts, Jose Figueroa, Michelle Hendricks, Megha Dabas, Eliza Macneal, David J. Community First Choice (CFC) allows states to offer attendant care services on a state-wide basis to eligible members, through their state Medicaid Service – in Colorado, Health First Colorado. Through CFC, members would have the option to direct their attendant care services or to receive services through an agency. Encouraging self-direction is a foundational program goal. To enable states to adopt CFC, Congress has lowered the percentage Colorado would have to contribute from the current 50% of program costs to 44%. Learn more about this program that is targeted to be implemented by 2025.
