Mitochondrial DNA Deletions in iRBD: Potential Marker but not PD Predictor Rapid eye movement (REM) sleep behavior disorder (RBD), also known as idiopathic REM sleep behavior disorder (iRBD), is a prodromal marker for Parkinson's disease (PD) and Lewy body dementia (LBD). While a proportion of iRBD patients develop PD or LBD, current diagnostic approaches lack reliable biomarkers for risk stratification. This study investigated mitochondrial DNA (mtDNA) deletions in cerebrospinal fluid (CSF) as a potential predictor. Interestingly, researchers observed elevated levels of mtDNA deletions in both iRBD subgroups – those who progressed to PD and those who remained iRBD – compared to controls. However, the current findings appear limited to predicting iRBD itself, and further research is necessary to translate this into PD risk prediction. Mitochondrial Dysfunction in REM-Sleep Behavior Disorder May Serve as Biomarker for Parkinson Disease: A recent study published in 'Movement Disorders' found that patients with idiopathic REM sleep behavior disorder (iRBD) demonstrated mitochondrial dysfunction in fibroblasts, which was associated with the early stage of Parkinson's disease (PD). The study included clinical evaluation of 28 individuals, showing deterioration in nonmotor scores for patients with iRBD compared to healthy controls. Mitochondrial dysfunction in iRBD patients suggests a connection to early PD and emphasizes the need for further research on predictive biomarkers. The study highlights the clinical relevance of mitochondrial changes in identifying individuals at risk of developing PD, emphasizing the importance of assessing mitochondrial function for early detection and intervention in neurodegenerative diseases.
