"We believe that unveiling the individual mechanical response to body position and PEEP may assist providers in tailoring treatment and optimizing lung function and protection for specific patients."   A recent study led by Drs. Morais, Alcala, and De Santis Santiago, just published in Critical Care Exploration, elucidates the global and regional mechanical responses provoked by prone positioning and Positive End-Expiratory Pressure (PEEP) in COVID-19-induced ARDS patients. Employing advanced respiratory monitoring techniques, such as Electrical Impedance Tomography and esophageal manometry, Dr. Lorenzo Berra's team revealed that a significant majority of patients exhibited a shift in global respiratory compliance after pronation, highlighting the growing importance of personalized approaches in managing ARDS. To navigate the intricate mechanics of body positioning effectively, the assessment of regional lung function becomes indispensable. In the relentless pursuit of improved patient outcomes, the Massachusetts General Hospital champions individualized care, recognizing that one size no longer fits all. Talk about an ethical nightmare in medicine... "I think the bottom line is that NRP violates foundational ethical norms around the determination of death and should not be pursued," University of Colorado bioethicist Matthew DeCamp told NPR It is certainly easy to agree with Mr. DeCamp. If doctors can restore blood flow by choice, and by choice restrict blood flow from the brain, did they really do everything medically possible to save you in the first place? If the tables were turned, though; if my child was on a transplant list with no available organ, it would be hard not to support NRP; especially if the result was a healthy organ for my child. I pray I never have to make that decision. Some additional thoughts: 1. There is already too much #GodComplex in medicine. 2. There is already too much "err on the side of profit" in medicine. 3. The difference between "prolonging life" and "saving life" may need to be considered. No matter how many days you add to a person's life, they will eventually die. 4. These days people cannot agree on the definition of life, as it relates to conception and birth. How are people going to agree on this? https://lnkd.in/g7xnPwZK

The importance of Phase IV studies is frequently neglected, however they have a fundamental role in evaluating efficacy and safety of interventions.
