Some good news for 2024! FOX31 KDVR.com reported the Colorado Department of Law awarded $4 million to state #opioidcrisis programs. Locally, Larimer County Sheriff's Office received $600,000 to tackle the opioid crisis at a systemic level with looks at policies, community support, organizational culture, intergenerational trauma and individual needs related to systems that lead to criminal behavior. Narcan saves lives. #narcan is the brand name of the generic #naloxone medication. When given in time, it can save a life by reversing an #overdose from opioids, including heroin, fentanyl and prescription medications. As someone whose work spans community and acute care, I know with certainty there are people we are not reaching with our current prescriber model. Delighted to be involved in the kick off of this series by Drug Policy Alliance. It has been a long time since I was in treatment for substance use disorder. A great deal has changed and a LOT hasn't. What would treatment be like that would make people want to go? What are the modalities that get people healthy and able to sustain wellness and recovery, as they define it? What are the most important elements of treatment for people with problematic use and what needs to be in place for people who use drugs (two different things, for sure). The World Has Changed—#340b Needs to Change with It When 340B was passed … • co-insurance costs weren’t at ridiculously high percentages, costing patients thousands of dollars—now 340B, as 46brooklyn Research says, is a program that “makes money off the backs of sick people” through exorbitant “cost-sharing”; • there was no #PartD program, so #Medicare didn’t cover those drugs at all, and didn’t, therefore, pay huge “spreads” on deeply discounted drugs; • roughly 200 hospitals were thought to be likely participants—now more than 2,600 are, including some that are part of institutions with endowments that exceed $40 billion; • Medicare paid hospitals for their outpatient services at their costs—now it pays 340B hospitals a huge spread (as high as 145% or more) according to MedPAC; • national pharmacies connected to dominant PBMs that are affiliates of dominant payers didn’t exist—now they suck billions out of the program; • roughly 250 million had health insurance—now more than 300 million do; and • the program was an incidental appendage of what was intended to be a massively larger Medicaid rebate program—now the 340B program is LARGER than the #MDRP and getting larger every day.
