The high level of medical complexity demands specialized care, including administering medications and managing intricate treatment plans. Caring for individuals with cognitive impairments, such as dementia, requires exceptional patience and adaptability to handle challenging behaviours and ensure safety. In end-of-life care situations, providing support and comfort to both patients and their families demands a high level of emotional intelligence. Home health care professionals often work in isolated environments, requiring resourcefulness and adaptability to various home settings. Communicating with patients and their families, navigating family dynamics, and addressing disagreements about care plans necessitate strong interpersonal skills. Nursing Facility national mandatory staffing rule has been finalized and will require facilities to provide, at a minimum, 3.48 hours of care per patient/resident day (ppd), with at least 0.55 hours ppd to be provided by registered nurses. FNs will have to provide RN staffing 24/7 - a significant challenge in many communities and especially in rural areas. The rule is expected to be implemented in phases, with temporary exemptions available to facilities demonstrating a good faith effort to hire staff. The rule is considered by many to be controversial - while intending to improve quality of care, it may result in additional cost of compliance for some facilities. Organizations who do not put the HEALTH of their OWN care teams FIRST are NOT SINCERE about the WELLBEING OF THE POPULATIONS they SERVE. 😒DISINGENUOUS MISSION STATEMENTS 😒 Due to their work demands, nurses are at high risk for disruption in their elimination patterns and menstrual cycles. These problems are COMMON in nursing: overactive bladders, urinary retention, urinary tract infections, irritable bowel, constipation, toxic shock syndrome, dysmenorrhea, and not to mention all the orthopedic conditions and mental stress and trauma which commonly correspond with nursing. These vicious cycle circumstances continue for nursing due to skeleton staffing, insufficient planning, lack of provision for adequate caregiver respite and uncompassionate leadership. Nurses in specialized areas are especially at risk because of the required lengthy orientation programs prior to independent work. 😒On a personal note, these are my most memorable moments in nursing: 😒I called into work only ONCE for menstrual cramps in my entire career. The received response was to come in anyways and walk off the gas pains. Three years later, I was diagnosed with stage five endometriosis. 😒I was pushed to return to work earlier than surgeons’ recommendations despite complications and extended stays. I was informed a CNO boasted about returning to work in 2 days after what she thought was the same procedure. My laparoscope was converted into a laparotomy. My path report was borderline ovarian cancer; my fertility chances were nearly nonexistent; and I experienced 3 complications and stayed in the hospital over a week. Also, I been criticized for not being willing to work with an abdominal drainage tube still in place and being unable to push the doctors to decipher primary from secondary hyperparathyroidism and evaluate kidney stones, cardiac arrthymias, bleeding tendencies, appendix/kidney/breast tumors/cysts, and ENT (and more) within a 2 week period I was forced to take off. 😒Both parathyroid and endometriosis conditions are plagued with myths and misunderstandings among providers. Therefore, I endured being belittled and humiliated as a hypochrodiac by both coworkers and some providers while being in survival mode. 😒There also needs to be more space to grieve losses. The 2-3 days “one size fits all” bereavement leave benefit is insufficient for any worker but especially for those who work in healthcare and first responder types of roles where death is a common occurrence. Why is this happening? It happens because there is skeleton staffing. Why is there skeleton staffing? Because we continue to lose trained nurses due to poor treatment, bullying, and burnout. I have to share a horrific story that a nurse recently told me. She was a traveler nurse in a highly acute and busy unit. She was in the middle of her menstrual cycle and needed to change her tampon. There were back to back cases and she asked her manager for a bathroom break. The manager responded: "I wish I could use the bathroom." No relief was offered and the next trauma rolled in. By the middle of the case she had bled through her scrubs. And her manager gave her a shrug of the shoulders after seeing the blood stain. To understand how selfless, exceptional, and amazing this nurse is--she said: "How do I ever put a child second to me....I had to stop n say, ok I know its all down my legs n my pants...but I just had to keep movin, kept workin...." This is a horrific story--but its not isolated. Why do 18% of nurses not make it past year one--nonsense like this! Why is this accepted in healthcare? Do you think if the CEO heard this story that manager would be punished? Unfortunately, this problem is bigger than just this one crappy manager. We have to do better. Two action items: 1) Leaders--you have a legal duty to assure work conditions do not violate your worker rights to sanitation. If staff cannot be relieved of their duties to use the toilet in a reasonable timeframe--chances are their rights are being violated. Develop protocols, standards and staffing to assure your organization is not breaking laws r/t access to a toilet. 2) Staff--support one another. Encourage each other to take breaks and step off the floor when permissible. No charge nurse should say--"Oh (eyes rolling) you want another break?" We have to change our culture from thinking breaks are a luxury to a necessity.
