While each survivorship care plan (SCP) is slightly different, the ideal SCP can help you understand the plan for your care after treatment, has strategies to manage short and long-term effects of treatments, facilitates accurate and efficient communication between members of your future medical team, and empowers you to have more control over your health. Patients receiving cancer treatment experience a high level of symptom burden. Symptoms can escalate quickly, and real-time response is essential to decreasing symptom burden, improving quality of life, and avoiding patients seeking alternative care sites. At Reimagine Care, we firmly believe that addressing symptoms before they escalate is crucial for enhancing the quality of life for patients with cancer. Our program focuses on proactive measures using technologies such as digital patient engagement platforms that provide electronic patient reported outcomes (EPRO), symptom reporting, automated coaching, and virtual care management can increase the timeliness of patient symptom reporting and access to clinician expertise ensuring that patients receive the support they need. After 7 years from the first day of recognizing palliative medicine, if I want to explain this field with just one photo, absolutely it is this photo. From my point of view, cancer is a perplexing thing in the world of medicine, even more than COVID19. Everybody who hears about cancer, even unsolicited , doesn’t want to listen again it . But once and for all, I should compose about that. Cancer is like a tree: the primary tumor is the tree root, and metastasis is the branch of the tree. Therefore, the role of palliative medicine for the patient will pass away soon. The light coming from the sky in this photo represents quality of life until death. There is a significant difference for cancer patients between being in the light and not being in the light. I contemplate that one day people will alter their beliefs about cancer and end of life. Please allow ourselves to recognize and be more aware of this field until the community can master their fear. Palliative care plays a crucial role in improving the quality of life for patients with serious illnesses. It addresses physical, emotional, and spiritual needs, working alongside curative treatments. Let’s continue to raise awareness and understanding about this essential field! Photo by: Amin fereshtian

"AMA: How can the medical community better support oncologists and patients? Dr. Barbara McAneny: Continue with early diagnosis. Stop telling people that cancer is a death sentence. Getting people over to see us and collaborating with us. Oncology—more than any other part of medicine—is a team sport. I can't do what I do without primary care doctors finding the cancers and sending them to me and pathologists reading the tumor type and geneticists helping figure out what the driver mutation is. And surgeons and pulmonologists and everybody else. There are a lot of people involved in managing cancer. One of the things that we really need to work on is better ways to communicate with one another. And unfortunately, the electronic medical record is a colossal flop for doctors communicating with each other. It's a great way to document a bunch of charges and make diagnosis codes.
