Identifying patients who have acuities that if managed with intensity and frequency will yield better outcomes and will reduce reduce higher medical treatment locations. I operated 40+ clinics in Florida. We developed a hospital at home program to help with our value based contracts. We identified high risk patient who utilized hospitals frequently for conditions that were preventable if proper steps would be taken to address their acuity in a home setting. Technology was essential to success. We used analytics to help identify what patients truly would benefit from the services. We focused on high risk COPD, HF, and in combination with with other conditions. Frail, ADL combined with the main acuity were also selected. The results we’re incredible. We reduced admits and visits by more than 60%. The patient results were more important. We regulated meds real time and engaged specialist when required to change meds. It was a game changer. As we were at risk we saw a reduction in avoidable cost and improved outcomes. Does it work? No MRIs Lol.

Why is this important:   Delayed discharge of patients from the hospital is costly to both patients and payers so planning early is important. Single measures of physical function at hospital admission are known to predict discharge disposition but the benefit of regularly updating those measures throughout hospitalization was unknown     Highlights:   -The results of this study indicate that regular assessment of patient mobility, as measured by the AM-PAC “6-Clicks,” improves discharge prediction  -Routinely collecting measures of patient mobility and using them with other patient information such as age and home living situation can improve discharge prediction and should be encouraged

One of the most influential and models of care that will lead #valuebasedcare is the #hospitalathome model of care. Once patients experience getting expertise medical care in the comfort of their own home, why would they ever opt to go into a hospital or doctor's office (unless they need to) ? This model of care will only continue to grow with #remotepatientmonitoring which will leverage #AI to help further improve the quality of care while lowering costs.
