Like many healthcare settings in the US, at PM Pediatric Urgent Care we’ve seen a spike in COVID-19 infections this summer. From the beginning of the pandemic, vaccine uptake has been below target in the pediatric population, and now that the updated formulation has been released for Fall 2024, it’s time to dial up the conversation with families about it again. Patients are likely to have questions. Is the vaccine right for my child? When should they take it? Do they need to take it at all if they had an infection? Family education is key to a strong and healthy relationship between pediatric healthcare provider and patient – how do you approach this conversation with your patients? https://lnkd.in/d42NMzMa

Researchers in Ohio studied 152 severely ill adult patients who were admitted to Ohio State University Hospital with Acute Respiratory Failure (ARF) between May 2020 and November 2022. Of these patients, 112 tested positive for COVID-19, while 40 did not. Among the 112 COVID-positive ARF patients, 20 had received COVID-19 “vaccines” and 89 had not. The death rate during hospitalization was 37% in the unvaccinated group, while it was 70% in the vaccinated group. The vaccine almost doubled the risk of dying from COVID-19 respiratory disease. The death rate was lower among all age groups if they were not vaccinated. The vaccinated group had higher rates of comorbidities (other health problems); however, even when the data was adjusted for this using the Charlson Comorbidity Score, the results still showed that being COVID-19 vaccinated carried a higher risk of death from COVID-19 respiratory disease. The researchers also compared antibody levels among the patients. The vaccinated group had higher levels of IgG4 antibodies, which are known to promote immune tolerance or antibody-dependent enhancement of the disease.

**Nirmatrelvir-Ritonavir No Aid for Long COVID Symptoms** 💊🔬😷 A recent study revealed that nirmatrelvir-ritonavir, administered for a 15-day course, does not alleviate long COVID symptoms at the 10-week mark in a vaccinated cohort. Despite its effectiveness in reducing hospitalization and severity of acute COVID-19, this antiviral treatment showed no significant benefit in addressing the prolonged symptoms experienced by long COVID patients. To date there are 10 confirmed measles cases in Florida….measles is a really, really scary disease. For those who are ill up to 3/100 will end up in a Pediatric ICU with either encephalitis or pneumonitis; and it has a per case fatality rate of from 1-3 per 1,000 cases. We have NO treatment for this virus and all that we can offer is supportive care….that’s it. There are 2 basic things that we can do, and the “surgeon general” of Florida is NOT advising that we do either of these things: 1) Is isolation. Measles has one of the highest R naughts, it’s the measure of infectivity, we know of…>12. That means every infected person will infect AT LEAST 12 others if not more. That is why we REQUIRE all suspected and confirmed cases isolate. Period. 2) The 2nd is to get vaccines to those who have not been vaccinated. That to date Florida’s Surgeon General has done neither of these two things is grossly negligent. To put this into context: if I did something similar in the ER, my license would be revoked. This is beyond incompetence and the current administration in Florida has proven that it doesn’t care about the lives of it’s citizens.

PSV20 HEs FOR OLDER ADULTS' PNEUMOCOCCAL PREVENTION. The objective of this study was to assess the clinical and economic impact of replacing existing vaccination strategies with the PCV20, which has the potential to reduce disease burden and associated costs. What was learned from the study? PCV20 was estimated to reduce invasive pneumococcal disease (IPD) cases by 3838, non-bacteremic pneumonia (NBP) cases by 10,380, and deaths by 1865 compared to PCV13→PPSV23; corresponding reductions for PCV20 vs. PCV15→ PPSV23 were 2775, 7803, and 1348. The use of PCV20 is projected to be cost saving among Argentinean adults, lowering total costs by $87.6 million and $80.8 million compared to PCV13→PPSV23 and PCV15→PPSV23, respectively. This economic evaluation of PCV20 will help inform decision-makers in Argentina and in other Latin American countries who will soon consider the use of novel pneumococcal conjugate vaccines
