For a long time, methadone was the only acceptable MAT in pregnancy. Now ASAM, ACOG & others recognize that buprenorphine is safe, and in fact, to basically continue whatever is working. But for a long time, there was a concern that naloxone could have adverse fetal effects, so the buprenorphine-only product (previously Subutex) was preferred to the buprenorphine/naloxone combination (Suboxone). Treating opioid use disorder (OUD) in pregnancy is something many providers are uncomfortable doing. The fact is, OUD isn’t going anywhere, so we need to educate ourselves on evidence-based care for this population. Massachusetts General Center for Women’s Health published new research comparing the safety of using buprenorphine (Subutex) or buprenorphine-naloxone (Suboxone) alone to treat OUD during pregnancy. Findings of note: 1. No significant difference in maternal morbidity, major congenital malformations, low birth weight, preterm birth, respiratory problems or delivery by c-section. 2. The risk of Neonatal Abstinence Syndrome was a significantly lower in infants exposed to buprenorphine with naloxone compared to those exposed to buprenorphine alone (37.4% vs 55.8%, respectively). This is a good argument for prescribing Suboxone vs Subutex during pregnancy.

A shift in testing for and managing gestational diabetes mellitus (GDM) to much earlier in pregnancy (before 14 weeks) can prevent health complications for both the mother and baby, according to a new series published in The Lancet. Series authors challenge the current approach to GDM management—which focuses on late GDM (24 weeks+)—and call for better detection and prevention efforts alongside a personalized, integrated life-course approach for those who experience or are at risk for GDM.
