Medically fragile patients experiencing homelessness all too often receive either a lower level of care (I.e.-congregate shelter, street outreach, etc…) or a higher level of care (SNF, long term care, etc…) than what is needed to support their illness because of the dearth of available resources to fill the gap. We need more programs like these that fill care gaps and to think more broadly about the impact they have on the health and well-being of our communities.
The recently released evaluation of Project Roomkey showed what is possible when there is political will, cross-sector coordination and collaboration, and person-centered approaches to housing and health care. In this new California Health Care Foundation blog, you can read about two Alameda County Project Roomkey 'sheroes' - Alexis Chettiar and Catherine Hayes. These amazing clinicians leveraged their Roomkey success and learnings to create, in collaboration with Five Keys Schools and Programs and other key partners, transitional and permanent housing for medically frail people. Katie Cornell is a patient services representative. Every day, she follows a strict schedule, picking up the clinic’s mail at 2 p.m. But on this day, something told her to head out a little earlier. She couldn’t explain why — just a feeling, a hunch. As she walked outside, she heard an odd noise coming from an awning that had been installed less than a year ago. It didn’t sound right. Instinctively, she banged on the window of a car in which a patient’s husband was napping while he waited. Startled, he woke up and moved his car. Seconds later, the awning came crashing down, right where the car had been parked. In that moment, Katie saved not only his life, but also the lives of three others, including her own. A routine task, a decision to step outside a little earlier than usual, taking action when something didn’t feel right — all are seemingly insignificant on their own. Yet Katie’s presence of mind, her awareness of her surroundings, and most importantly, her heart to care for those around her, converged to create a moment of extraordinary impact. Later, Katie sent her manager a simple text: “I saved a life today.” Thank you, Katie. Your story is a reminder that every decision we make matters. We each have the power to make a difference. It’s about being aware, being present, and above all, caring deeply for the people we serve and for each other. One of the most devastating experiences for residents of long term care facilities is medication error: wrong dose, wrong medication, or wrong resident. These can be deeply disturbing events for residents and their loved ones and also for the care professionals involved, who are officially labeled "perpetrators." Have you ever experienced a medication error in a health care setting? What was it like? Medication errors consistently rank in the top ten compliance citations issued by the State of Oregon -- and likely many other states. Why do they keep happening? In my experience, it's the day to day chaos and constant interruptions on the ground that distract care staff and cause these adverse events. Care professionals need strong processes in place that reduce chaos and increase focus and attention so that they can check, double check, and document every medication that they administer. We created #TheLiveWellMethod to do just that -- to help communities build strong, repeatable practices of quality improvement.
