We have personally seen these same outstanding outcomes, savings, and feedback from patients and their families. As a hospitalist, I have serious concerns regarding the transformation of hospital purpose into profit generation versus provision of the best care possible. This drive for profit has turned ER's into through-put operations in which a multiplicity of tests is ordered up front, whether the patient actually needs them or not. Hospitalists now often focus on checking the boxes in the EMR rather than on whether or not they actually did what was in the boxes. Profits rise, patient safety issues abound. I struggle on a daily basis with ethics and integrity of care in a healthcare system that asks me to create profit based on the appearance of care. https://lnkd.in/gZ_RCnu5

EM plays an important role in supporting the management of these events whether it’s coordinating essential resources, working with leaders and clinical staff to find solutions for optimizing flow, and implementing contingency plans or alternate care spaces which can ensure effective response and patient care. This expertise and the ability to convene interdepartmental teams helps to mitigate the impact of overcrowding and maintains the overall functioning of health care facilities. The horizontally and vertically integrated continuum of EMs at the facility/system, local, regional, and state government levels works to ensure care can still be provided appropriately but, as with anything in this world, it’s not without distinct challenges. 💫

Of course, a hospital’s reach and impact extend beyond its walls and into the surrounding neighborhoods and communities. Each community may have different needs, and those needs can change. Whether providing transportation for patients to and from an appointment or hospital stay, offering education and employment opportunities, providing affordable housing — and so much more — all hospitals and health systems provide innumerable benefits to their communities. Today is a good Monday. We have 18 open spots out of 24 for patients to be seen today. As we continue to improve I wanted to share our latest progress here as we work to improve how we provide primary care to our patients. For those of you who have been following along our team has made a significant and concerted effort to improve our access and the experience of care for our patients. We currently serve over 2000 patients and families in our practice and over the last 6 week cycle we have made a concerted effort to make our office more easy to access, a calmer workplace for our employees and a place where we effectively team together. What did we do? - install a doorbell for patient check ins to limit interruptions for our secretary - work as a team to triage all patient concerns rather than delegate this to a single team member - emphasize that prebooked appointments are for care that can only be received in person (well baby visits, chronic disease, antenatal visits) - work to optimize all visits before they are seen in clinic: this means lab work or imaging done before we see patients in some cases to ensure we can optimize our time in person - instill the principle of acknowledging receipt of requests vs feeling pressure to complete them the moment they are received - emphasizing to our team that their time and health is critical to our success and that being calm is more important than having an inbox at zero. - we have opened nearly every day to be fully accessible by online booking and patients are able to book as it works for them. Do I anticipate some hiccups into the future - it's likely.
