The Long Term Care Community Coalition released a report with federal data on five-star ratings, ownership type, enforcement, and other important information for the nearly 15,000 U.S. #nursinghomes in the federal #CMS dataset. Here are their key findings: ➡ One in four U.S. nursing homes (24.6%) is a Problem Facility ➡ For-profit nursing homes are much more likely to be poor performers ➡ Nursing homes that are part of a chain have lower #staffing and ratings ➡ Nearly 9% of facilities (around 1,350 facilities) have an #abuse icon https://lnkd.in/gcGPS6xR

There are people, post Covid ,who want to be nurses at the bedside. They have been met with multiple barriers. There are also people, post Covid, nursing students, who also want to work at the bedside,who struggle with finding human resources within their nursing programs. Bedside nursing is where some people who want to be nurses want to go. Every nurse to be doesn’t want to be a nurse practitioner. Every nurse to be doesn’t want to own their own Medspa. Every nurse to be doesn’t want to run their own nursing business. Every nurse to be doesn’t want to be a CRNA. Every nurse to be doesn’t want to be a travel nurse. Let’s address this issue by removing barriers that make it more difficult for people who want to be nurses, to be nurses. #thoughtleadership #nursingeducation

Inpatient care is in a state of emergency. I recently caught up with a doctor friend of mine who runs a hospitalist program at a large West Coast AMC. He’s even-keeled and certainly not one to catastrophize. But when discussing inpatient care, he pulled no punches, explaining, “It’s absolute chaos. At least during COVID, we had a mission.” Why? The work is harder than ever. Patients are sicker and have more complex needs than ever before. Yet there aren’t enough healthcare workers – especially bedside nurses and care coordinators – to care for them. And unless patients can be discharged home, there’s often nowhere to send them. Under-staffed skilled nursing facilities are also full. Consequently, care is happening in slow motion. Patients without close advocates (family/friends) literally get lost in the shuffle. Even those with advocates can spend days in the emergency department awaiting a bed. Unfortunately, this is becoming the national norm. We can't ignore it. When I asked my hospitalist friend how he’d try to fix these problems, he said he’d invest all available capital in staffing. We must always be thinking about the #rootcause of the problem. And, in healthcare, you will often have to follow the #money to find the root cause. Yes, patients are sicker, and staffing and capacity are poor. But, if we followed the money, we would likely find things like: - #Insurance companies denying needed care - Hospitals not staffing to patient #acuity - #Nursing care being wrapped into the room rate - And the list goes on and on We have to fix the #brokensystem if we want to see better patient outcomes. Where’s the staffing tree?? 🌳 Nursing homes are doing everything they can to hire a skilled workforce amid an ongoing labor crisis. If you’ve worked in healthcare (or any other job) you know that a federal staffing mandate won’t make new workers appear! However, there is concern that it might create a shortage of available SNF beds and decrease access to healthcare for a growing population of seniors. Don’t throw the baby out with the bath water! I agree that the root cause of inpatient medicine’s problem is that hospital systems are understaffed. We need to invest more in staffing, including nursing and most importantly social workers. The social workers on the 18th floor at Virginia Mason in Seattle were amazing when I worked as an medical intern. We need to invest more in education. More nursing schools and more medical schools need to be built. Also, we need to encourage more people to go into medical profession. We need to highlight how truly special and privileged it is to be in the medical field. We need more people going into primary care for prevention and becoming internist to coordinate the care of our sickest patients. But technology can be used as a stopgap as we try to recover our crumbling healthcare infrastructure. Technology can also be instrumental in ushering our healthcare system into the relatively new millennium.

Last week, LeadingAge President and CEO, Katie Smith Sloan, joined the growing number of #longtermcare experts speaking out in opposition to the looming #federalstaffingmandate. In a detailed letter to President Biden, Sloan reiterated that nursing homes and the federal government share a common goal of improving quality of care in nursing homes, but emphasized that mandating minimum staffing levels alone will not lead to meaningful quality improvements without enough qualified workers to fill open positions. Sloan proposed a variety of solutions to address the issue and suggested an industry roundtable to discuss creative solutions across of variety of LTC stakeholders. "Residents, patients, providers, regulators, and policymakers all have a part to play in this effort."
