Flow diversion technology has truly revolutionized how we treat ICA aneurysms, while most of my colleagues in the field would likely have the same treatment strategy, one of the questions that comes up for discussion in our field is the effectiveness of flow diversion in treating aneurysms that share an origin with an artery such as the ophthalmic, choroidal or PICA. We have always seen excellent results even in such cases with no issues/recurrences. Sharing a case out of quite a few in my practice with an ophthalmic artery origin aneurysm where the artery arose from the aneurysm neck. We opted for endovascular treatment using a single flow diverting FREDX stent MicroVention-Terumo 6 month followup angiogram demonstrates complete obliteration (resolution) of the aneurysm with a patent ophthalmic artery and excellent stent patency. Such patients are continue on a baby aspirin for 2-3 years then sometimes are able to come off all blood thinners(off label). All done transradial ofcourse and without a single incision ! #cerebralaneurysm #flowdiversion #endovascular

ICG-PEG12-Osu is a Cyseries dye. Indocyanine green (ICG) is a anthocyanin dye used for medical diagnosis. It is used to determine cardiac output, liver function, and liver blood flow, as well as for ophthalmic angiography. It has a peak spectral absorption close to 800nm. Can some early THA revisions be avoided? Novikov et al in 2019 concluded that “A surprisingly large proportion of acute revision THAs are potentially avoidable". Of a total of 60 revisions (51.3%) were potentially avoidable, as defined by Shackford et al. All of these avoidable revisions were caused by either suboptimal cup positioning, intraoperative fracture, early aseptic loosening or gross implant under-sizing with subsidence of the femoral component, or leg length discrepancies (>1cm)¹. Heyyy Connections.. Let me explain one of the surgical procedure which I have seen in my Clinical posting.. Latarjet procedure is a possible surgical procedure to treat patients with anterior shoulder instability and accompanied bone loss. It involves transferring the coracoid process and its attached conjoint tendon to the anterior glenoid rim.  The conjoint tendon acts as a sling to the inferior subscapularis and anteroinferior capsule when the arm is abducted and externally rotate. The addition of bone to the glenoid rim increases the anteroposterior (AP) osseous diameter. The inferior capsule is reinforced with a portion of the coracoacromial ligament. #article #shoulderinstability #latarjetprocedure

The glabella zone carries a high risk of blindness whilst performing HA filler injections for frown lines. Most notably, the supratrochlear artery, a direct branch of the ophthalmic artery, ascends from the muscular layer of the medial eyebrow along the medial canthal vertical line of the intercanthal vertical zone. The dominant paracentral artery may cause ocular complications during injections of glabellar lines between the medial eyebrows, especially at the radix vertical lines. Extreme care was undertaken in this case. My clinical pearls: Use a medium-density filler, stay superficial, either intra-dermal or sub-dermal, compress the artery as you retrograde, tent the needle upwards, aspirate (if you believe in aspiration) and very low-pressure on the plunger.
