Vital signs should be the first and last data set addressed and documented by the care team in the Emergency Department and any critical care setting. This basic feature of the patient evaluation has been largely tossed to the curb in favor of reliance on high tech diagnostic lab and imaging tools. Attributing tachycardia solely to patient anxiety is a cognitive error that can result in failure to diagnose serious conditions including sepsis, necrotizing fasciitis, pulmonary embolism, internal hemorrhage, and many others. Adherence to the "Tips to Avoid Vital Sign Errors at the Time of Discharge" provides valuable suggestions to overcome the problem of discharging patients with VERY abnormal vital signs!
