Here they were lucky so, nothing goes wrong but don't take a risk. 🙏 Make sure this activity is covered in HIRA and clearly mentions maximum stacking heights or number of boxes in controls, Residual risk should be in a low or medium category after applying appropriate controls.⛑️ If this activity is covered in HIRA and appropriate controls are defined then this was a behavioural issue and need to identify the root cause of what triggered him to overstack. 👷 What could be the probable cause for this unsafe act.?
