Value-based care considers patients’ health-related social needs. For example, when a veteran with #COPD struggled to get his wife, who uses a wheelchair, to her medical appointments, Clarion Hospital--a participant in the CMS Innovation Center’s Pennsylvania Rural Health Model--stepped in to help. A care transition navigator connected the couple to a program called Ramps for Hope, to build an outdoor wheelchair ramp so that the husband could get his wife safely out to the car by himself. Medically fragile patients experiencing homelessness all too often receive either a lower level of care (I.e.-congregate shelter, street outreach, etc…) or a higher level of care (SNF, long term care, etc…) than what is needed to support their illness because of the dearth of available resources to fill the gap. We need more programs like these that fill care gaps and to think more broadly about the impact they have on the health and well-being of our communities.
The recently released evaluation of Project Roomkey showed what is possible when there is political will, cross-sector coordination and collaboration, and person-centered approaches to housing and health care. In this new California Health Care Foundation blog, you can read about two Alameda County Project Roomkey 'sheroes' - Alexis Chettiar and Catherine Hayes. These amazing clinicians leveraged their Roomkey success and learnings to create, in collaboration with Five Keys Schools and Programs and other key partners, transitional and permanent housing for medically frail people. Questioning the Adequacy of One Level 1 Trauma Center in Westchester County: Is it Enough? As a healthcare leader with a passion for optimizing patient care, I can't help but wonder: Is one Level 1 Trauma Center truly sufficient for the needs of our community? Let's dive into the data: According to recent research, Westchester County relies heavily on Westchester Medical Center as the region's sole Level 1 Trauma Center. While this center undoubtedly provides critical care services, questions arise about its capacity to effectively serve the entire population of our county and neighboring regions. 🏥 Consider this: With a population of nearly 1 million residents in Westchester County alone, along with the influx of visitors and commuters, the demand for trauma care services is substantial. Factor in the geographic spread of our communities, and accessibility becomes a pressing concern. 🌟 It's not just about numbers; it's about access to timely and high-quality care when every second counts.
