Take a few minutes and participate in this important survey!
Good morning #AlabamaNonprofits! If you have ten minutes this week, please consider participating in the following survey from Alabama Department of Public Health (https://lnkd.in/gQZGqDSK). ADPH seeks to partner with its community network to understand more about Alabama's perception of health. The Community Health Improvement Survey asks participants about their community and areas to address health concerns. This initiative helps ADPH write its State Health Assessment and deliver services better to low resource areas across the state. You have an important perspective on the needs in your community, and, for those in our St. Louis market, our colleagues at the St. Louis region hospital systems would like to learn from you. The systems represented below are working together to learn from community members to identify the top health concerns, to determine priorities regarding health-related needs, and to develop solutions designed for St. Louisans. The survey will take about 5 minutes. All responses are confidential and anonymous. You will not be asked for your name, and only combined results will be shared. Social determinants of stress affect everyone's health, but the impact is not equal. Taking this survey--and sharing it with diverse networks--is an opportunity for more equitable healthcare resources.
Community Health Equity Survey (CHES) is now live - spread the word! DPH created the CHES survey to collect information directly from key stakeholders and community members that will be used to help communities improve conditions that impact health, particularly those most impacted by health inequities. DPH will use survey data to allocate funding, improve programming, and develop policies that address health inequities. Spread the word to older adults in your community to take the survey. https://lnkd.in/eBW65J8H #Massachusetts
