METHYLENE BLUE Uses:Treatment of methemoglobinemia 🔹 Adjunct in the treatment of refractory septic shock 🔹 As a dye in medical procedures and surgeries, including parathyroid gland delineation 🔹 Treatment of ifosfamide-induced encephalopathy Dose: 🔹 Methemoglobinemia: 1-2 mg/kg IV over 5 minutes; may repeat after 1 hour if needed. (Max 7mg/Kg) 🔹 Septic Shock (Off-label use): 1-2 mg/kg IV bolus 🔹 Parathyroid Delineation: 5 mg/kg IV administered over 10 minutes before surgery. Presentation: 🔹 Injectable solution: 5 mg/mL Note: 🔹 Mechanism of Action: Methylene blue acts as an electron donor in the reduction of methemoglobin to hemoglobin, and it also inhibits nitric oxide synthase and guanylate cyclase, which can be useful in vasodilatory shock. 🔹 Onset and Duration: Onset of action is within minutes for methemoglobinemia, with effects lasting up to several hours. 🔹 Side Effects: Can include nausea, vomiting, headache, dizziness, and in high doses, serotonin syndrome or hemolysis in G6PD-deficient patients. 🔹 Contraindications: Contraindicated in patients with G6PD deficiency , to the risk of hemolysis.

🔎 💊 European Medicines Agency safety committee, the #PRAC, at its July meeting has recommended new measures to minimise the risk of aspiration with GLP-1 medicines during anaesthesia and deep sedation. Patients treated with GLP-1 medicines should inform the doctor involved if they have a planned surgery. The PRAC also agreed a direct healthcare professional communication (DHPC) for glatiramer acetate, a medicine authorised for the treatment of relapsing forms of multiple sclerosis. Following an EU-wide review of all available data, the PRAC concluded that the medicine is associated with anaphylactic reactions which may occur even after months up to years after initiation of treatment. Cases with a fatal outcome have been reported. The DHPC will inform healthcare professionals about this risk and recommend that patients and/or caregivers be advised of the signs and symptoms and to seek emergency care in the event of an anaphylactic reaction.
