Termed as continuity of care, this model not only enhances patient health but also alleviates the workload for family doctors. Key Findings: 🔍 Improved Patient Health: The study indicates that maintaining a long-term relationship with a single doctor enhances patient health outcomes significantly. 📉 Reduced Doctor Workload: Contrary to common assumptions, the research reveals that having a consistent GP actually reduces the workload for family doctors. According to the study, patients who consistently saw the same GP experienced an 18% longer interval between visits compared to those who consulted different doctors. This finding challenges traditional perceptions and underscores the value of continuity of care in modern healthcare systems. The implications of this study are profound, emphasizing the importance of fostering lasting doctor-patient relationships. We're at a crucial point in redefining how medical teams operate to enhance patient outcomes and alleviate the pressure on our healthcare systems. Project Objective: We're excited to launch a groundbreaking project aimed at enhancing medical team performance by focusing on three critical yet often overlooked factors: cognitive diversity, psychological safety, and motivation. Our hypothesis: improving these areas will significantly uplift patient care quality and ease the healthcare burden. Methodology: This ambitious 18-month project will involve periodic evaluations of these key areas at the beginning, and then at 6, 12, and 18 months. This strategic approach allows us to monitor progress and gauge the impact of our interventions on team performance and patient care. Intervention: We invite medical teams to participate in a series of innovative activities, including workshops, training sessions, and practical exercises. These are designed to bolster cognitive diversity, foster a psychologically safe work environment, and enhance intrinsic motivation within teams. Expected Impact: We're aiming for a transformative impact on patient care quality. By addressing these 'invisible forces,' we expect to see improved team communication, heightened efficiency in managing complex cases, and a reduction in staff stress and fatigue. Our ultimate goal is to establish a scalable and sustainable model for ongoing healthcare improvement. Decoding the persistence of delayed hospital discharge: An in‐depth scoping review and insights from two decades comes from the DeGroote School of Business - McMaster University Alyaa Abdelhalim Manaf Zargoush Norm Archer & Mehrdad Roham They did a review of systematic reviews over the last 24 years and the PRIMARY Causes of delayed discharges are: The primary causes of Delayed Hospital Discharge (DHD) identified in the review include: 1. Lack of Rehabilitation Services: The unavailability of appropriate rehabilitation services necessary for patients' recovery post-discharge. 2. Hospital-Based Delays: Inefficiencies within the hospital system, such as bed shortages and administrative hold-ups. 3. Awaiting Transfer: Patients often have to wait for transfer to other hospitals or care facilities, causing significant delays. 3. Faulty Organizational Management: Inadequate discharge planning and transfer of care issues due to poor organizational management. 4. Age-Related Factors: Older, frail, and dependent individuals face complex health needs that make them particularly vulnerable to delays. 5. Inadequate Access to Care: Barriers such as limited access to residential care, domiciliary support, and insufficient hospital capacity hinder timely discharge. 6. Poor Communication and Coordination: Deficits in communication and coordination of care, including poor clinical handover practices, contribute to DHD. 7. Funding Difficulties: Financial constraints and challenges in implementing financial integration can delay necessary post-discharge services. 8. Cognitive Impairment: Patients with cognitive impairments often experience longer hospital stays due to the need for specialized care and suitable discharge destinations. 9. Socioeconomic Factors: Patients’ socioeconomic status can impact the availability and coordination of post-discharge care, leading to delays. https://bit.ly/3YtwHJw
