📣 We have a simple mantra here at Get Well: what is best for the patient, is best for the client, and what is best for the client is best for Get Well. Start with the patient and work your way out from there; it will never lead you astray. Because of this mantra, when we launched our new SDOH screening and navigation program, we strategically included it as part of our core systems with our innovation partners. The results have been nothing short of amazing. 💜 Every day, our internal Slack channel called "Patient Love" highlights stories from the frontlines and the impact we are having. It is a constant reminder of why we do what we do and how technology can impact patients and communities. The SDOH Screening and Navigation program screens every patient and member through a multimodal approach and has teams of Navigators who call patients and members to ensure they have the resources needed to live the healthiest and most fulfilling lives possible. This ultimately leads to better health outcomes and stronger communities. 😇 Here is an amazing snip from our internal Slack channel by (the absolutely incredible) Taryn Fuerst, who engaged with a patient who needed help. 💪 This is the hard work, but it's the right work. It's the most impactful work and we are moved every day by stories like this.

** What's our patient engagement 30 days after program start? ** ** Did our preventable readmission rate improve in the last 6 months? ** ** Do patients show a meaningful improvement in a target outcome pre- and post treatment? ** It's hackathon week at Awell! 🤖 5 days of working on a moonshot project with a cross-functional team and the goal to ship a feature by the end of the week⚡ Like everyone at Awell, I love the hackathon atmosphere of intense collaboration, short deadlines and thinking without constraints 🚈 Our team decided to help care teams define and track metrics such as the ones mentioned above, and how changes to their care flows impact those metrics💡 We also decided to "build in public" so you'll see updates on our progress all through the week. Today was spent on: 1. Nailing the problem definition 2. Exploring the solution space 3. Defining elements of a solution we could build in a week 4. Identifying dragons 🐉 - the major risks / hairy issues we might run into 5. Assigning DRIs (directly responsible individuals) for the different aspects of what we need to build Excited to have the amazing Abigaila E. Just arrived at the Patient Centricity & Engagement conference and listened to the first talk delivered by Shannon Altimari. One of the things that stood out to me the most is that: “Informed patients report feeling more empowered, and empowered patients have better outcomes”. Really fascinating to hear how GSK are gaining more insight from patients and involving them in the decision making that shapes clinical trials. Interesting to hear that their approach is to see patients as we do KOLs in terms of the value they can add.  #PatientFirst #PatientCentricity #PatientEngagement
