Two years into the #CalAIM initiative, those tasked with implementing the program continue to run into challenges. While some progress has been made around enrollees receiving more comprehensive care, many others enrolled in California’s Medicaid program are not being effectively connected to or enrolled in the newly available services. #SDOH and #HRSN data can help. These data help to clearly identify gaps in existing programs – as well as who would benefit the most – and pinpoint ways in which specific members’ health can be better supported. 62% of states do not take advantage of Medicaid 1115 waivers to test innovative SoDH strategies. Does yours? For my state, let's just say my representative is due to get a letter from me. ;-) As 55% of our health outcomes are determined by conditions in which we are born, grow, live, work and age, i.e. Social Determinants of Health (SoDH). Medicaid, which, by design, serves primarily low-income populations with greater social needs. Therefore, isn't Medicaid one best agencies to help address SoDH. States just need to take advantage of CMS's framework of coverage including 1115 demonstration waivers. Lets work together to ensure all have access to healthy outcomes! Check out if your state is utilizing 1115 waivers here: https://lnkd.in/gMh7MKPk. Take a look at the options for coverage of SoDH programs in the Nov 2023 CMS framework here: https://lnkd.in/gCZ3eeFh

Many states have taken action to help people get health care again during the Medicaid unwinding, but there’s far more progress that needs to be made. 23% of people who were disenrolled in Medicaid unwinding are still uninsured, according to a recent KFF survey. Policymakers can help their constituents to live a healthy life by expanding Medicaid, staffing Medicaid assistance centers in their states, and passing continuous enrollment for children. https://lnkd.in/esDNdfWr

Over the past several years, #MedicalRespite care has made significant progress in several states toward becoming a benefit Medicaid will pay for. This month on A Closer Look, our policy blog, we dive into this issue and take a look at two new issue briefs on the subject. The first brief — "Status of State-Level Medicaid Benefits for Medical Respite Care" — provides a current snapshot of state-level activity in the 14 states already paying for medical respite care through Medicaid — or actively getting ready to do so. The second brief — "CalAIM Implementation of Recuperative Care Services: Lessons Learned" — covers what's working well with California's implementation of the CalAIM recuperative care service, what remains a challenge, and advice for other states looking to add a statewide Medicaid benefit for recuperative care. The number of people enrolled in both Medicare and Medicaid increased from 10 million in 2016 to more than 12 million in 2022. Learn more about ACAP’s work with the Medicare-Medicaid Integration Alliance to improve health care for people who are dually eligible. 👇 #Medicare #Medicaid #DualEligible #IntegratedCare
Recent data from The Commonwealth Fund highlight the growing population of individuals who are dually eligible. As this population continues to grow, we're pushing for person-centered solutions to integrated care. Approximately 12.8 million Americans are dually eligible in 2024. This population typically has greater health care needs, yet faces poor coordination that leads to worse outcomes and higher costs for taxpayers. Key facts about individuals eligible for both Medicare and Medicaid: • 26% have five or more chronic conditions • They generate 1/3 of Medicare & Medicaid spending • Federal spending for this population reached $440.2 billion in 2019 It’s crucial that we continue to advocate for policies that improve health outcomes and quality of life for individuals who are dually eligible. We invite you to join the conversation with us and learn more about MMIA at https://lnkd.in/d6GgNPh9.
